The role you'll contribute:

The Care Navigation Lead is responsible for assisting in coordination of care and services for assigned patients under the direction of Care Management Leadership, and in collaboration with the multidisciplinary team and patient/family. The Care Navigation Lead establishes a general working knowledge of payor-specific documentation, regulatory and authorization requirements, the clinical review process, provision of acute and post-acute services, as appropriate for the role. Actively participates in outstanding customer service, escalates as appropriate to effectively and promptly resolve issues, and accepts responsibility in maintaining relationships that are equally respectful to all.

The Care Navigation Lead of the post-acute team ensures discharge arrangements are compliant with payor requirements and regulatory guidelines. Responsible for assisting in implementation of patient discharge plans as directed, assisting with coordination of clinical data collection, and ensuring third-party payer notifications and authorizations are secured and documented as a support function for Care Management staff.

The Care Navigation Lead acute care and/or ED team establishes a general working knowledge of post-acute appointment needs. Ensures regulatory requirements are compliant with CMS and AHCA regulations and post-acute appointments are secured prior to patient's discharge, as appropriate within their role.

Qualifications
The expertise and experiences you'll need to succeed :
  • Minimum qualifications :
    • High School Diploma or GED
    • At least five-years of experience in a customer service environment
    • At least three-year experience in a healthcare environment
    • Ability to multitask and function in a stressful, fast-paced environment
    • Professional presentation and decorum aligned with environment in which they are situated.
    • Ability to speak using clear and professional language.
    • Demonstrate written communication skills.
    • Demonstrate interpersonal/verbal communication skills.
    • Knowledge of medical terminology
    • Knowledge of Microsoft Office (Word, Excel and PowerPoint)
