JOB SUMMARY:

Responsible for exemplifying the UTHSCT's Mission, Vision, and Values, providing outstanding customer service the Lead Community Health Worker (CHW) will function as a member of the team that conducts family support and connections with resources to help families and their children achieve good health practices and for children primarily ages 0-3 with resources to achieve milestones through developmental stages of determined ages. Primary responsibilities include community outreach and awareness, family recruitment and follow-up, facilitation of health improvement, health promotion, health program development and support, prevention activities, and collaboration with regional and local partners within the community. This position works as an interdisciplinary team member and reports directly to the Project Coordinator. This position does require evening, weekend, and travel for outreach, recruitment, and health promotion. Performs other related duties as required/assigned.

MAJOR RESPONSIBILITIES / DUTIES / CRITICAL TASKS:
• Train (new) CHWs and offer guidance.
• Complete documentation and required reporting for all services in specific database within given timeframe.
• Participate in community outreach and deliver/assist with screenings in the community.
• Provide one-on-one education and encourage healthy parenting and behaviors with families.
• Conduct family home visits, when necessary, i.e., follow-up, health education, application assistance, etc.
• Assess and assist families for potential eligibility for grants, programs, and/or community resources.
• Assist families in completion of forms/applications required to receive needed services.
• Assist families in scheduling future appointments and linking to providers and/or needed services.
• Provide families with referrals to external community resources (such as food pantries) as needed.
• Provide follow-up when working with families within the given timeframe.
• Speak with/present community groups/organizations and diverse populations about the program in a sensitive and culturally competent manner.
• Assist with development, improvement, and standardization of processes and guidelines.
• Collaborate with other team members and stakeholders to promote population health.
• Handle all phone calls, emails, and in-person interactions cordially and professionally.
• Remain current on early childhood issues, chronic disease and behavioral health trends, business practices, and health education and promotion theory.
• Communicate effectively with team, the Center, and UTT HSC colleagues, project partners, stakeholders, subcontractors, and consultants to achieve outcomes, metrics, and improvement targets.
• Coordinate with director supervisor and/or assigned leadership educational and outreach events focusing on project goals and requirements.
• Handle administrative functions to promote, coordinate, collaborate, and support the scope of work required to achieve stated goals.
• Use excellent oral and written communication skills to prepare memos, formal correspondence, acknowledgements, notes, and reports to interact with team members efficiently and effectively, internal, and external partners to accomplish essential tasks.
• Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the art practices; participating in professional societies as deemed appropriate.
• Handle confidential/sensitive information with discretion and adherence to HIPAA rules.
• Performs other duties as assigned.

Qualifications

REQUIRED EDUCATION / EXPERIENCE:
• High School diploma or GED required.
• Current Community Health Worker certification by the Texas Department of State Health Services required. Must maintain current CHW certification.
• Two (2) years of healthcare and/or early childhood related experience.
• Two (2) years of experience conducting and promoting health education preferred.
• Basic knowledge of standard office equipment such as: telephone, facsimile, copier, personal computer, printer and audio/visual.
• Basic knowledge of resources available for families (state/federal/local assistance programs, bright by text, brain gym, help me grow, etc.)
• Experience working with daycares.

REQUIRED LICENSE/CERTIFICATION:
• Current Community Health Worker certification by the Texas Department of State Health Services required. Must maintain current CHW certification.
