Job Description

SCOPE OF POSITION
The position of Lead Patient Care Coordinator holds a pivotal responsibility in upholding the seamless coordination of patient care services within Ballad Health's Health Improvement Team. The Health Improvement Team is a multi-disciplinary team that focuses on improving the quality for patients within our Value Based Contracts and enhancing coordination of care to reduce unnecessary duplication and expenditures across the health continuum. This role will assist operations management by actively contributing to leading a team of patient care coordinators, overseeing the processes related to quality assurance and care transition, and engaging in collaborative efforts with medical personnel to ensure the delivery of exemplary patient experiences. Notably, this position will encompass a portion of the conventional patient care coordinator duties while encompassing an expanded leadership purview.
Responsibilities:
Team Leadership: Provide leadership and guidance to a team of patient care coordinators, including training and daily oversight of a team of coordinators.
Communication: Promote effective communication among quality and transition of care coordinators to ensure a shared understanding of department goals.
Quality Assurance: Monitor team adherence to procedures to maintain high standards of patient care and satisfaction.
Problem Solving: Address patient concerns and collaborate with the healthcare team to ensure smooth patient flow.
Data Management: Utilize electronic health records (EHR) systems to manage patient information. Regularly assess data to determine progress towards department goals.
Training and Development: Train new patient care coordinators, keep the team updated on industry regulations, and facilitate skill enhancement.
Collaboration: Work closely with coordinators and Director to ensure a cohesive approach to patient care.

EDUCATION AND EXPERIENCE
Required:
  • Graduate of an accredited LPN program
  • At least 3 years of proven experience as a patient care coordinator or in a similar role within a healthcare setting.
  • Familiarity with quality improvement initiatives and Value Based Contracts
  • Strong understanding of medical terminology, healthcare regulations, and patient care processes.
  • Excellent leadership and interpersonal skills.
  • Proficiency in using electronic health records (EHR).
  • Exceptional organizational abilities and attention to detail.
  • Outstanding communication skills, both written and verbal.
  • Ability to handle sensitive information with confidentiality and professionalism.
  • Problem-solving aptitude and ability to work well under pressure.
    Valid and active LPN licensure in appropriate state
