Summary

Assesses patients. Assists patients and families with their discharge planning. Interacts with the acute care social workers prior to the transfer as needed. Assists patients and their families to achieve and maintain maximum functioning in and outside the unit through needs assessment, crisis intervention, counseling and information and referral. Performs other miscellaneous duties as needed.
