A Licensed Practical Nurse (LPN) working in Home Health provides nursing care to patients in their homes. The following is a general job description for an LPN in Home Health:
Provide patient care: The LPN is responsible for providing direct patient care, including wound care, administering medications, and monitoring vital signs.
Collaborating with the interdisciplinary team: The LPN must work effectively with the interdisciplinary team, including physicians, registered nurses, therapists, and other healthcare professionals, to develop and implement a care plan for the patient.
Assessing patient conditions: The LPN is responsible for assessing the patient's condition and reporting any changes or concerns to the appropriate staff.
Administering medications: The LPN is responsible for administering medications as prescribed, monitoring for adverse reactions, and documenting medication administration.
Educating patients and families: The LPN is responsible for educating patients and their families about their health conditions, treatments, and self-care.
Maintaining accurate records: The LPN is responsible for maintaining accurate and complete patient records, including nursing notes and progress reports.
Complying with infection control practices: The LPN must comply with infection control practices to prevent the spread of diseases.
To be eligible for the position, an individual must be licensed as an LPN in the state in which they are practicing. They must also have relevant experience in home health or a related field, as well as strong clinical skills and the ability to work independently. Good communication and interpersonal skills, as well as a patient-centered approach, are also critical for success in this role.
This is a not full time but contracted travel position.
