We are looking for an LPN to join our Outreach home programs here at Penn Medicine at Home. The schedule is Monday-Friday 8:30 am- 5:00 pm, no weekends. This candidate must have at least 2 years of home health experience.

We are looking for an LPN to join our Outreach home programs here at Penn Medicine at Home. The schedule is Monday-Friday 8:30 am- 5:00 pm, no weekends. This candidate must have at least 2 years of home health experience.
Summary:
- Responsible for supporting the initiatives of Penn Medicine at Home Programs. Assisting with patient needs for various programs such as Senior Health Link, THRIVE, and initiatives. Coordinate and collaborate with Penn Medicine at Home community partners to align services, such as including the participating schools of nursing, community outreach centers, and organizations. Responsible for carrying out Penn Medicine at Home community education and health promotion for the various programs.
Responsibilities:
- Plans, implements, and evaluates all community based health promotion programs for the Senior HealthLink program.
- Manages and provides education, supervision, and counseling for SHL outreach site clients and families regarding care needs as appropriate to each client's condition.
- Communicates with, or supervises student and faculty in communicating with physician regarding client's condition. If possible, arranges home care services with physician, referral source, and clients consent.
- Maintains ongoing relationships with the business community for purposes of determining and meeting health care needs.
- Coordinates health education and screening programs for the agency. Provides information concerning the Senior HealthLink program and community resources.
- Prepares and maintains client records and related reports.
- Works with other community agencies and promotes good community relations through involvement in community health programs and events.
- Reviews and makes recommendations to leadership regarding Senior HealthLink outreach educational needs and budget.
- Supervises nursing students caring for clients in conjunction with nursing faculty. Orients and inservices Senior HealthLink nursing school faculty.
- Evaluates, and regularly reevaluates the health care and nursing needs of the clients. Initiates, implements, and makes necessary revision to the client's plan of care per Senior HealthLink protocols.
Education or Equivalent Experience:
- Licensed Practical Nurse in state of Pennsylvania (Required)
- Car Insurance (Required)
- Driver's License (Required)
- 3 years of experience with at least 2 years of home health LPN experience (Required)
- 4 years of progressive experience planning and facilitating a community or outreach program required (Required)
