Job Duties

Southeastern Virginia Training Center is seeking an individual to support the facility's mission by providing LPN care for individuals living in SEVTC Steppingstones neighborhood. The incumbent will be responsible for medication administration, collaborating with the nurses and the ID team to provide optimal services, and providing ongoing training to direct care staff in the proper medical care and services received.

• Administers medications and provides medical treatment in compliance with policies, procedures and evidenced based nursing practices.
• Observes individuals for change in Health Status and gathers data, evaluating safe preventive, routine, emergency and follow-up care to individuals which are consistent with the person centered approach, policies, procedures and evidence based nursing practice. To report change in health status to RN on duty.
• Perform venipuncture/Collect labs as ordered by the MD/FNP.
• Obtains and prepares lab specimens as needed
• Completes and assures accuracy and timeliness of required documentation including but not limited to ID notes, Facility Event forms, Nurses Daily Report sheet, and MAR's per facility policies and Medicaid guidelines by the due date.
• Assures that all required documentation, including DNR if applicable, copies of MARs, Treatment sheet, PRN sheets are checked and supplied to another medical facility when the individual is being transported to that facility.
• Responsible for reviewing and assuring accurate transcriptions of medical orders by the end of the shift.
• Keeps medical services staff, supervisor and RN on duty informed of changes in an individual's condition.
• Responsible for checking and maintaining emergency equipment monthly in all assigned areas.
• Initiate requests to participate in job related educational programs based on an assessment of own knowledge, skills, and abilities (assessment performed in conjunction with RN and/or other unit nurse supervisor.

Minimum Qualifications

Requires a valid Virginia Practical Nursing license or licensure in a participating state in the Nurse Licensure Compact, as a Licensed Practical Nurse.
• Current certification in CPR and first aid.
• Successful completion of/ability to complete pre-service training class, including completion of medication course, CPR, first aid, and state approved behavior interaction procedures.
• Valid Driver's License
• Skills in performing patient assessments, medication administration, tube feedings, sterile and non-sterile treatments, suctioning, tracheotomy care and catheterizations.
• Ability to carry out nursing care procedures including the physical ability to turn, bend & lift individuals without restrictions.
• Ability to communicate effectively both orally and in writing with supervisors, PST, direct care staff, parents/AR regarding health care services and needs.
• Ability to recognize signs and symptoms of illness with people with disabilities and ability to monitor their own health care needs or communicate problems.
• Skill and knowledge of medication administration.
• General knowledge of medical conditions, lab work results, and medications.
• General knowledge of the principles of behavior and their application to training for persons with intellectual disabilities.
• Demonstrated ability to train direct care staff as needed.
• Ability to pass medications accurately.
• Ability to summarize data and write concise progress reports.
• Ability to verbally present objective review information to the PST (Personal Support Team), the community and individuals' family/authorized representatives.

Additional Considerations

• Minimum of one year experience working with persons with severe Intellectual and developmental disabilities.

These positions are hourly/wage and not eligible for benefits. Hourly (wage) employees may not work more than 29 hours a week and are limited to working 1500 hours in a year.
