Position Summary:
The Logistics Coordinator/Case Aide operates under the direction of the Director of Community Health Systems to assist with specified back-up case management activities, including intake/referrals, program information and paperwork verification. This position is professional and requires the employee to exercise appropriate discretion and judgment in the performance of his/her assigned duties and responsibilities.

EXPERIENCE REQUIREMENTS
CNA or previous related job experience.

ADDITIONAL REQUIREMENTS
  • Become AIRs A/D certified when CRIS offers testing.


SKILLS AND KNOWLEDGE
Excellent verbal and writing communication skills.General knowledge of health settings, healthcare issues, and experience working with clients in a home setting. Ability to work independently.Knowledge and expertise with computer software applications and Word processing programs. Must enjoy working with the elderly.

Essential Functions:
  • Provides phone support for all CCU incoming calls, potential, and existing clients.
  • Provides clerical support to the Director and CCU Case Managers by assisting with mailing, fax transmittals, copying and filing.
  • Corresponds through phone and email with INH Providers, Nursing Facilities, SLPs, Hospitals, EHRS Providers, Managed Care Organizations (MCO's) and Bridge Coaches.
  • Documents phone calls and case note phone calls in Case Aide on S:Drive.
  • Creates and maintain client list for each case manager.
  • Schedules all Care Coordinator appointments
  • Creates and maintains new filing system as well as digital files.
  • Corresponds with nursing homes and VA for UPS in Vermilion County.
  • Understands existing/new policies and regulations set forth by IDoA.
  • Sends Community Care Program applications to those who request and to interested participants or referrals.
  • Assists with MCO transfers and correspondence with BEAM.
  • Submits MCO transfers to respective MCO.
  • Maintains filing for CCU.
