Job Description

Scope of Position
The LPN - Adult Day Care Coordinator, Madison House will oversee admission inquiries, completing tours, assessments, admission paperwork, qualifying clients for VA and/or Medicaid Waiver Choices benefits programs, knowledge of adult days care state regulations, signing contracts, arranging transportation to/from center. The Coordinator will need knowledge of Adult Day Care Policy and Procedure. The Coordinator will work closely with all outside entities related to parient care. This individual wll work directly with the elderly and their caregivers. Additionally, the Coordinator will be responsible for working in the program areas as needed, assist in dining room, assist with all aspects of patient care and activities of daily living. The Coordinator will be involved in marketing and developing referral sources, attending expos, fairs, and siminars.

Education and Experience
Graduate from an accredited LPN program. One to two years clinical experience in assisted living and adult day care preferred. Knowledge and ability to perform various clinical and general office procedures. Preferably experience working with the geriatric population.

• Valid and active LPN licensure in appropriate state
• CPR required
