Position Description: To perform a wide variety of nursing, supervisory and clerical duties. To be responsible for total resident care and supervision of nursing duties during shift.

Essential Job Functions:
  • Delegates responsibilities for direct care of specific residents according to needs.
  • Review and revises resident's care plans.
  • Assists in admitting residents, observes and evaluate transfers of hospital residents.
  • Assures that necessary paperwork is completed by the direct care staff.
  • Maintains medical record documentation on every resident to assure accuracy and consistency of nursing care provisions.
  • Contact physician(s), family members or appropriate individuals in regards to any incidents involving respected resident.
  • Supervises nursing assistant on duty at same the time.
  • Pass medications as needed.
  • Prepare and follow through on providing treatments to residents.
  • Call for emergency assistance when appropriate.
  • Call administrator on any incidents that happened on the shift.
  • Call Director of Nursing Services for inquiries regarding proper treatment for residents and as needed.
  • Assures that nursing staff on the floor are appropriately assigned and that resident care is being done according to nursing policies and procedures.
  • Acts as a leader to the nursing staff by teaching, demonstrating and orientating as necessary.
  • Assists in setting up resident appointment with medical providers.
  • Notifies families of resident appointments and outcomes at appointments.
  • Assists nursing assistant with direct cares for residents.
  • Informs the Director of Nursing Services of changes as they occur with staff and residents.
  • Understands and abides by facilities emergency procedures.
  • Processes physician orders.
  • Completes and sends faxes and contacts physicians regarding resident health issues and change in resident conditions.
  • Understands and follows safety policies and procedures and educates direct care staff in safe work habits.
  • Attends in-service education to maintain their nursing license status.
  • Ensures completion of resident and/or employee incident reports and follows up as necessary.
  • Participates in resident discharge process when necessary.
  • Monitors and ensures that infection control practices are being followed appropriately.
  • Works with other staff members and department heads in a positive manner.

Non Essential Job Functions:
  • Assist in preparing for physician rounds.
  • Assists in completing physician paperwork after rounds.
  • Assist in arranging transportation for residents to and from appointments.
  • Assist with scheduling and filling the schedule when needed.
  • Assists in keeping nursing inventory on the floor at an appropriate level.
  • Assists in completing chart audits when necessary.
  • Function as the building supervisor when needed.
  • Participates in facility committee meetings.
  • Assists in preparing for resident care conferences.
  • Assist in preparing rooms for new residents and/or re-admits.
  • Push and pull wheel chairs.
  • Assist nurse aides with two person's transferring.
  • Answer telephones.
  • Assist in passing trays and delivering room trays.

Qualifications:
  • Knowledge of computers.
  • Must be able to communicate effectively with staff, residents and others in a professional manner.
  • Able to work as a team member with other employees.
  • Able to work independently.
  • Must be able to follow instructions.
  • Registered with the State of Minnesota in good standing as a LPN.
  • Certified in CPR.
  • Experience in long term care or we will train!
