The role you'll contribute: The Specialty Efficiency Care Coordinator will focus their work on providing patient-centered care as part of the medical neighborhood. They will utilize targeted patient registries and other performance data to assist with planning and managing patient care in collaboration with our health partners. The SECC will provide comprehensive care coordination services that promotes continuity and minimizes fragmentation of care within the healthcare delivery system. This role will support key strategic activities that drives the rapid advancement of equitable outcomes through accountable, high-quality, affordable, whole-person care.

The value you'll bring to the team:
  • Performs post-visit outreach for patients with complex needs to coordinate follow-up care that maximizes efficiency. Outreach may also include assessing and addressing self-management, social determinants, and other potential barriers to meeting treatment and lifestyle goals.
  • Identifies and proactively outreaches patients due for needed services including follow-up visits, labs, imaging, procedures and assists with coordinating care as needed. This includes queuing up orders to providers, scheduling appointments, and working with primary care to ensure that co-managed patients receive timely preventive services.
  • Provides health coaching support in collaboration with the practice care teams by engaging and educating patients on self-management tactics that promotes optimal health and safety.
  • Uses payer and other performance data to assess and address gaps in medication adherence, HCC/RAF, clinical quality, cost, utilization and educates care teams of opportunities and strategies to improve performance.
  • Assists in connecting patients to additional resources including pharmacy, care management, community resources and other social services with an emphasis on whole-person care.
  • Contacts patients and families to evaluate their status following discharge from an ED or hospital and ensures follow-up care is highly coordinated.
  • Serves as a liaison by relaying information necessary for continuity of care to other health professionals and agencies including closing the referral loop after a patient misses or cancels an appointment to prevent delays in diagnosis or treatment.


Qualifications
The expertise and experiences you'll need to succeed:

Minimum qualifications:
  • Graduate of an accredited school of nursing
  • Licensed Practical Nurse (LPN)
  • 3 years of healthcare experience
  • Understanding of value-based care concepts
  • Licensed Practical Nurse (LPN)
  • CPR Certification

Preferred experience:
  • 5 years of healthcare experience
  • Ambulatory office, care coordination, care management, or health coaching experience
