Why does the job exist?

The LPN Supervisor - MDS coordinator completes MDS assessments for all residents of a long-term care facility. Care plans are required to be completed within a specific time frame in accordance with state and federal regulatory guidelines. After the assessment is completed, the MDS coordinator creates care-plan reports for the staff of the long-term care facility to follow. They monitor and ensure care plans are adhered to, properly documented, and residents receive the best care. MDS Coordinators must be able to work overtime, weekends, holidays or flex hours as needed to complete tasks.

How does it get done?

The LPN Supervisor - MDS coordinator is tasked in completing various MDS paperwork; submit them on time and make certain it meets with the current guidelines and regulations as these regulations may vary each year.
Ensure completion and submission of MDS data within regulatory time frames. Conduct/coordinate care plan meetings with appropriate staff/residents/families/POAs. Prep for and attend all required meetings.

Who are the customers?

The primary customer are the brave men and women that answered our nations call, executed their duties and now are unable to live in their respective communities due to failing health. This job will also support the widows of the service men and women to include parents that have lost children in combat (Gold Star Parents)

Ideal Candidate

Must possess RAC-CT
Two years experience completing MDSs
Two years experience developing care plan

Minimum Qualification

Licensure as LPN or LVN, and two (2) years of experience as it pertains to the essential duties and responsibilities of the classification.

Employment Requirements

This position is designated safety sensitive. Employment is subject to pre-employment and random testing, in accordance with all terms and conditions of federal and state law, rules, and regulations in relation to alcohol and/or drug testing.

Must possess and maintain a valid American Heart Association BLS / CPR Certification

Employment is subject to criminal background check pursuant to New Mexico Caregivers Criminal Screening Act, (Sections 29-17-2 through 29-17-5 NMSA 1978 and 7.1.9.1 through 7.1.9.11 NMAC) and is conditional pending results.

Successful completion of the Health Insurance Portability and Accountability Act (HIPAA) training.

Must sign and observe confidentiality agreement.

Must be able to read, write, speak and comprehend the English language.

Occasional travel for training & meetings and driving a State of NM owned motor vehicle maybe required; Working hours may include weekends, holidays, overtime, and callback hours. Essential functions of the job and specific physical ability requirements will be available at the time of interview or may be obtained at the New Mexico Veterans' Home Human Resources office.

Working Conditions

Work is performed in a long-term care facility and office setting with exposure to visual/video display terminal (VDT) and extensive personal computer and phone usage. Walking, standing, lifting, pushing, pulling, sitting and bending over are routine physical efforts required by the job. The ability to lift up to fifty (50) pounds and the use of various types of medical equipment, computers and manual documentation. Sitting at a computer and entering data is necessary at times. Use of copier, telephone, and fax machine. Incumbent may be required to drive a State-owned vehicle. Incumbent may be exposed to physical and verbal abuse from residents, moderate noise levels and stressful situations. Health hazards include routine exposure to resident illness blood and bodily fluids. and potential for chemical sensitivity exist related to repeated us of latex gloves, disinfectant agents, wound dressing medications or chemicals. Overtime is an essential function of this positions.
