Schedule: Full-time, Exempt

Shift: Days, 8:00 am - 5:00 pm

Location: AdventHealth Daytona Beach - position will be remote with some on-site expectations

The role you'll contribute:

Under the direction of the Thoracic Program Leader, the Lung Nodule Nurse Navigator is to ensure cancer patients are effectively and efficiently navigated through the healthcare system receiving appropriate physician, support, and educational services throughout their journey. Responsible for establishing, building, and maintaining strategic relationships between our community physicians and their office staff.

The value you'll bring to the team:
  • The focus of this role will be to manage and follow up on incidental lung nodule referrals, high-risk lung nodules found incidentally through Epic dashboard and from lung cancer screenings.
  • Facilitates patient continuity of care between hospital departments and provides patient follow-up for laboratory, radiology, and pathology procedures. Timely identification and resolution of issues are handled effectively and in a timely manner; as it relates to the delivery of patient health care and physician relations
  • Coordinates and participates in weekly thoracic tumor board and lung nodule clinic. i.e., tracks scheduled procedures follow pathology results, coordinates with pathology/radiology, and monitors outcomes of meetings.
  • Collaboration with ordering providers to coordinate additional workups for patients based on treatment planning- including diagnostic procedures and outgoing referrals.
  • Understand and follow Oncology Lung Navigator Workflow Standard Operating Procedure.


Qualifications
The expertise and experiences you'll need to succeed :

Minimum qualifications:
  • Graduate of a school of Nursing.
  • Current valid State of Florida license as a Registered Nurse
  • Current valid Driver's license
  • Basic Life Support (BLS) certification
  • Minimum of one year Oncology and/or thoracic experience

Preferred qualifications:
  • Bachelor's degree (BSN) or Master's degree (MSN) in Nursing
  • Current certificate in Oncology nursing
