DESCRIPTION

The LVN Nurse Navigator will manage pre-procedure phone intake, provide pre-op and post-op patient instructions and follow up. The LVN nurse navigator will be responsible to follow up on with surgical teams for pre/post op care as needed as well as managing escalating any pre-op or post-op patient triage needs to RN Nurse Navigators or ambulatory services RN.

The LVN Nurse Navigator needs to have strong clinical and communication/documentation skills to collaborate and coordinate with multi-disciplinary health care teams in ensuring optimal care for patients and families in busy perioperative arena, able to effectively manage large patient case load and be adaptable to program changes. Teamwork, communication, initiative, and creativity are essential to the success of this Perioperative Services Nurse Navigator. This telephone perioperative care navigation will be provided to patients receiving care from multiple service lines at various locations. Nurse navigator will work in a team compromised of 3 RNs and 2 LVNs and complete pre-op/post-op calls for periop services operating rooms and procedure spaces.

MINIMUM QUALIFICATIONS
  • Licensed Vocational Nurse (LVN) - issued by the state of California.
  • BART or BLS at time of hire with commitment to get BART w/in 6 months of hire date.
  • Two (2) years of relevant experience with recent ambulatory telephone call management
  • Knowledge of universal precautions and proper use of body mechanics. Required
  • Ability to take initiative, prioritize critical demands, exercise independent judgement and work in a team environment.
SPECIAL CONDITIONS
  • Must be able to work various hours, days, shifts, on-call and various locations based on the 24-hour Medical Center's business needs.

Pay Transparency Act

Annual Full Pay Range: $71,410 - $88,782 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $34.20 - $42.52

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
