Position Summary

Access East, Inc. is a care management and practice support company providing comprehensive complex, high/rising risk, and low risk management to over120,000 covered lives. We are a 501 (c)(3) entity that supports Medicaid, Medicare, Self-insured and Uninsured populations. Services include care management, pratice support, annual wellness visits, health coaching, free clinic wrap around services, screenings/care gap closures, and SDOH coordination, just to name a few.

Management accountability and decision-making authority for determining the strategies, interventions and activities that will be effective and appropriate to further enable the capability of the function. This role serves inpatient care management for Access East, Inc. which could include collaboration with external organizations. Accountable for ensuring performance of the program in accordance with goals and stewardship responsibilities. Communicates key management decisions and informs department leadership of issues affecting program and system performance effectiveness. Facilitates development of and adjustments to work plans, work processes, budgets, staffing levels, and other resources needed to deliver services that meet expectations of the program. Facilitates change to advance the organization and uses resources and operational practices to drive execution of plans. Ensures that policies, procedures and practices are operationalized to support safe, reliable care, regulatory compliance, and fosters a continuous learning culture. Accountable for a positive organizational culture in decision making and performance management to influence work attitudes and enhance job satisfaction. Supports quality programs such has infection prevention, regulatory compliance, performance improvement and/or patient safety. Key liaison between the leadership teams and staff of the health care facility and programs.

Minimum Requirements

Bachelor's degree in Nursing or Master's degree in Social Work is required. Master's degree is preferred.

3 to 4 years of experience is required.

Case Management Certification preferred.

Other Information

Position manages and oversees daily operations of the Oncology Navigation program.
