MDS Coordinator

Bayshore Residence & Rehabilitation is recruiting for an RN MDS Coordinator to become a part of our team!

Shift: Days

As an MDS Coordinator, you will be responsible for overseeing and assuring completion of the RAI/MDS process. This position is responsible for the Medicare and MDS process. An ideal candidate will appreciate a collaborative team of professionals whose primary focus is caring for the individual and touching the lives of families. If you are a motivated caregiver who knows the importance of providing excellent care to our residents and patients, you will thrive working with our multidisciplinary team.

Additional responsibilities of the MDS Coordinator include:

• Providing a final quality check prior to MDS transmission.
• Directing and assisting resident or patient and family education with coworkers.
• Orientating new nurses to the RAI process and providing retraining when necessary.

Additional requirements of the MDS Coordinator include:

• Current active and valid Licensure in Minnesota
• Certified through AANAC as a Resident Assessment Coordinator (RAC-CT) or ability to achieve upon employment.
• Experience with MDS 3.0 and current Medicare guidelines.
• PR Certification or ability to achieve upon employment.
• Excellent communication and hospitality skills.
• Basic computer skills.
