Job Summary: Functions under the supervision of the Director of Nursing and carries the responsibility for completing the nursing sections of the MDS+ assessment for and coordinating its completion with appropriate participation of all other disciplines. Completes all MDS+ information coordination to allow the final report to be completed and submitted on time.

DUTIES AND RESPONSIBILITIES:

Demonstrates Competency in the Following Areas:

Representative Duties: (including but not limited to)
  • Develops a schedule prescribed by regulation for the IDCP team to complete timely and accurate assessments.
  • Shall complete sections of the MDS form using sources of information such as the residents record, the residents, his/her family, direct care staff, residents physician, licensed professionals from all disciplines and consultants
  • Must sign and date the end of the assessment at R-2B to certify that all assessments are complete
  • Attends weekly medicare meetings and presents patient status, progress and assist in discharge planning as well as any follow up needs or items.
  • Attends other facility requested meetings per Administrator requests and reports to team based on parameters and objective of the meeting.
  • Serves as a liaison between rehab department and center staff, physicians as well as families presenting a courteous, compassionate and respectful manner at all times.
  • Responsible to perform QA and report to QA committee per ADM. Collaborates with facility leaders in developing, implementing, reviewing, revising and monitoring facility wide performance improvement activities.
  • Ensures timely and accurate completion of MDS and CAA's per MDS regulations
  • Participates in facility orientation program as requested.
  • Participates in and plans inservice programs directly related to MDS services as well as center regulatory education required for compliance.
  • Ensures timely submission of all MDS assessments to CMS.
  • Ensures work areas are kept clean, neat and safe environment
  • Attends and participates in workshops, seminars, etc. to keep abreast of current changes in the industry as well as to maintain a professional status.
  • Maintain confidentiality of all pertinent patient/resident care information to ensure that patient/resident's rights are protected. entiality
  • Demonstrates the ability to anticipate customer needs and to respond effectively and efficiently.
  • Complies with all safety and compliance protocols and programs as well as company policies and procedures

Professional Requirements:
  • Adheres to dress code, appearance is neat and clean.
  • Completes annual education requirements.
  • Maintains regulatory requirements, including all federal, state, local regulations and accrediting organization standards.
  • Maintains and ensures resident confidentiality at all times.
  • Reports to work on time and as scheduled.
  • Wears identification while on duty.
  • Attends annual review and performs departmental inservices.
  • Works at maintaining a good rapport and a cooperative working relationship with, departments and staff.
  • Represents the organization in a positive and professional manner.
  • Attends committee, CQI and management meetings, as appropriate.
  • Resolves staff concerns at the departmental level, utilizing the grievance process as required.
  • Ensures compliance with policies and procedures regarding department operations, fire safety, emergency management and infection prevention and control.
  • Effectively and consistently communicates administrative directive to staff and encourages interactive departmental meetings and discussions.
  • Complies with all organizational policies regarding ethical business practices.
  • Communicates the mission, ethics and goals of the facility, as well as the focus statement of the department.
  • Promotes a positive work environment and supportive of co-workers

Regulatory Requirements:
  • None Required.

Skills:
  • Basic computer knowledge.
  • Good oral and written english communication skills
  • Must be a Registered Professional Nurse with a current license to practice in NYS
  • RAC - certification is highly encouraged
