A large healthcare system local to South Jersey is looking to add a Medical Care Coordinator to their Comprehensive care team. This person will be joining a team that provides care management services to HIV positive patients in an urban population. They will be responsible for managing 50+ cases developing client care plans, monitoring patient progress, help identify support services for patients such as housing, transportation, counseling, testing and any additional support services based on acuity. They will be responsible for documenting patient progress with the goal of improving patients medical history and future health and well being. They will be responsible for documenting patient records in Epic.

Required Skills & Experience

Bachelors Degree

3+ years of medical case management/coordination experience

Experience completing case management assessments and develop client service care plans such as housing, transportation, counseling services etc

Experience/knowledge of available financial and social service resources in the PA and NJ Regions

Nice to Have Skills & Experience
