Minimum Qualifications

Requires technical knowledge in the field acquired through an apprenticeship, a 2-year degree, or professional certificate. Please contact your Human Resources office for an evaluation of education/experience in lieu of the required minimum qualifications.

Preferred Qualifications

UHC Minimum:
  • Associate or vocational degree in healthcare, nursing, or similar profession.
  • Knowledge of case management principles.
  • AHA Heartsaver CPR/AED certification required. Can obtain after start date.
UHC Preferred:
  • Bachelor's degree preferred.
  • Previous work in clinical case management, a healthcare/integrated care setting, and/or college health is preferred.
  • Familiarity with health care accreditation (Joint Commission or AAAHC) is preferred.
  • Demonstrated history and experience working with individuals from traditionally underserved populations.


Position Summary

Reporting to the Associate Director of Integrated Care, the Medical Case Manager is an integral member of the Integrated Care Team and provides appropriate and individualized case management to students and eligible partners. The Case Manager facilitates continuity of care using interprofessional collaboration and coordination of appropriate health care services and community resources across the care continuum. Responsibilities include oversight of internal and external referrals from medical or nursing services, as well as providing back-up to the CAPS case managers. The Case Manager will follow professional and organizational standards for case management and documentation. This position maintains collaborative relationships with all UHC departments, Student Care and Outreach, as well as other campus and community partners.

Case Management for Referrals
  • Collaborates with the interprofessional team to effectively implement care coordination while maintaining privacy, fiscal accountability, and advocacy.
  • Serves as a liaison between UHC and community providers to assist in obtaining past treatment records, scheduling appointments, and obtaining referral records.
  • Assists with referral tracking for clients referred from UHC medical clinics to internal and external providers.
  • Builds pathways for referrals to and from UHC.
  • Identifies barriers that may prevent the patient's ability to carry out the plan of care, such as lack of resources or access to care.
  • Maintains a record of relationships and resource connections for use by UHC colleagues.
  • Ensures that records and data are maintained in accordance with organization policies and federal and state regulations.
  • Works collaboratively with Student Care and Outreach Case Managers to address health-related social needs.
  • Assists CAPS case management team in transition of care appointments and referrals as needed.

  • Serves as a pivotal agent of the interprofessional health care team by collaborating with internal team members, leading teams, educating patients and staff, as well as communicating with all stakeholders across the continuum of care.
  • Communicates relevant information to the patient/client and interprofessional health care team across the care continuum.
  • Applies effective teamwork and collaboration skills to overcome identified barriers to produce quality and effective patient outcomes.
  • Supports patients/clients for ongoing, longitudinal individualized plans of care and follow-up plans of care with the aim of preventing adverse outcomes related to health and academic success.
  • Disseminates pertinent information to members of the interdisciplinary team.
