Medical Director - Hospital at Home

Lee Health is looking for a Medical Director for our Hospital at Home Program!

This is a hybrid position consisting of 30% Administrative and 70% Clinical Responsibilities.

The Hospital @ Home (H@H) Medical Director will collaborate to design and lead the long-term vision, strategy, and implementation of H@H and be responsible for clinical oversight related to system success in meeting H@H goals. The H@H program will build home-based and virtual services to provide clinical care that would traditionally be provided to various acute hospital-based inpatients. The Medical Director will be responsible for areas including, but not limited to, quality, safety, pharmacy, ancillary services and capabilities, clinician and patient experience, and seeking guidance of subject matter experts for Emergency, Medical, and Surgical at home.

The Medical Director must demonstrate a deep understanding of hospital-based care and the ability to translate that knowledge and experience to establish hospital-level care in a home setting. In home care is inclusive of clinical, operational, regulatory, and financial leadership. The Medical Director will collaborate with the Hospital at Home Leadership team to execute the H@H operational model at launch, during program evolution, and through continuous optimization of the Lee Health H@H care delivery operations. This will include the identification of patients eligible for care through the H@H Division, improving access to home-based care, maximizing facility capacity, and delivery of cost-effective care while maintaining service and quality on the same level (or higher) as a traditional hospital-based inpatient setting.

JOB REQUIREMENTS

Education Requirements: Medical Degree, MD or DO required.

Experience Requirements: Minimum of 5 years of clinical practice after residency training and/or population health management experience.

State of Florida Licensure Requirements: Licensed Physician in the State of Florida

Certifications/Registration Requirements: Board Certified in applicable specialty or subspecialty

Other:

Administration experience strongly preferred, experience with cross-functional teams and projects, knowledge and experience with contracts with various risk types.
