Job Description

Under direction of the Chief Executive Officer and system-wide Chief Medical Officer, The Chief Medical (CMO) Officer of this long-term care skilled nursing facility will serve a dual role with the primary responsibility being the planning, review, supervision and control of the medical and health services of the facility. Secondary responsibility will involve medical oversight in key areas for the Post-Acute Care Service line. The CMO will formulates policies, methods and procedures, and prescribes standards for treatment, diagnosis, control and prevention of illness, as well as, supervising methods of delivery and evaluating patient care. Collaborates with the President/Chair of the Medical Board and Chief Nursing Officer (CNO) on clinical and administrative issues, and reports regularly at Board meetings. The Chiefs of the clinical services, where applicable, report administratively and clinically to the Medical Director. Ensures optimal utilization of resources and compliance with all applicable government and NYC Health + Hospitals rules and regulations and medical standards of care, to improve outcomes and the patient experience

Specific Functions:

Facility
  • Organizes and coordinates physician services and the services provided by other professionals as they relate to patient care and will oversee the quality of radiology, pharmacy, and lab services. Oversees the surveillance and promotion of employee health, safety, and welfare.
  • Participates in administrative decision making and recommends and approves relevant policies and procedures and will be responsible for communicating issues and concerns to system clinical leadership when warranted.


  • Establishes and implements policies, procedures, and guidelines designed to assure the provision of adequate, comprehensive services and will also monitor, evaluate, and improve performance of medical services through performance improvement program.
  • Demonstrates proficiency in the knowledge of regulatory compliance based on, however not limited to the New York State Department of Health, Center of Medicare and Medicaid Services that relate to patient care and related services.
  • Works with medical staff and administration to ensure high value and high quality of care is delivered to patients with a focus on improving the patient experience, improving the health of the population, and maintaining fiscal responsibility and will be a part of the multi-departmental approach in improving the case mix index.
  • Collaborate with system physician leadership on new programs and their integration into the post-acute care service line.
  • Actively participates in the clinical marketing of Nursing Facility services within NYC Health and Hospitals.
  • Provides direct oversight of utilization management activities including Medicaid, Medicare and managed care appeals, medical necessity under various admission and continued stay criteria.
  • Coordinates primary, sub-specialty and behavioral health provider services and medical care in the facility
  • Coordinates the review process prior to granting or renewing professional privileges or association, of any physician, provider, dentist or podiatrist
  • Implements resident medical care policies and collaborates with the health care team including pharmacist, dentist and other clinical consultants ensuring that polices are followed and highest standard of care is provided.
  • Guides providers and other health care professionals to provide person-centered care that meets relevant clinical standards; Is involved in designing and implementing training programs relevant to patient-centered care.
  • Oversees the Infection control program ensuring that rules and polices are followed and implemented at the highest standard.
  • Leads quality assurance and performance improvement projects
  • Helps the facility educate and train its staff in areas that are relevant to providing high quality patient care
  • Ensures attending physicians, practitioners and other direct reports are informed and provided updates on federal and state regulations as well as internal policies and procedures and pertaining to nursing home rules and regulations involving patient care.

• Provide clinical care to residents, as needed
• Keeps up to date on clinical knowledge by ongoing professional development through self-directed and continuing education

Service Line
  • Lead the development, evaluation, and continuous improvement of clinical practices, protocols, and patient outcomes for the service line.


  • Provides oversight for medical staff activities in accordance with state and federal regulations, rules and code, review of internal policies and practices, ensuring consistency in interpretation and administration across facilities.


  • Monitor and manage key clinical and productivity performance measures relevant to service line and health system priorities


  • Create clinical programs in Post-Acute care, and develop training programs for medical providers focused on quality improvement and medical standards of care
  • Mentor, guide, and support medical staff, fostering ongoing education and professional growth.
  • Forge and maintain relationships with hospitals, health systems, and other medical establishments to promote collaborative endeavors


Minimum Qualifications

Minimum Qualifications:
1. Valid license issued by the New York State Education Department (NYSED) to practice Medicine; and,
2. Current Board Certification in Medical specialty; and,
3. Five (5) years of clinical leadership experience.

Departmental Preferences:
• Long Term Care Medical Leadership Experience required
• Geriatric Board Certified, AMDA Certified Medical Director, Palliative Care Certification, preferred
• Proficient in utilization of EMR systems and Microsoft Office Suite
• Demonstrated ability to take initiative and exercise good judgement with staff, patients, system stakeholders and other health care/community partners.
