Summary

Position Summary

The Medical Outreach Case Manager (MOCM) is a professional social services position that is responsible for the provision of case management services and achievement of positive intervention outcomes for assigned clients. He/she works ongoing with patient caseloads, delving deeper into more medically and socially challenging issues and health needs, addressing barriers for patients. Medical Outreach Case Managers may meet patients on-site at various locations - in the hospital, at the bedside, at a local community agency, in a patient's home, etc. to assist with navigating the system, providing education and advocacy and assisting with applications, goals, and completion of paperwork. He/she will ensure that client service delivery practices are ethical, culturally competent, respectful of diversity and inclusion, welcoming, and geared to customer service satisfaction. This position will work in collaboration with various departments across the MaineHealth system and outside agencies to achieve the desired outcomes for our clients.

Required Minimum Knowledge, Skills, and Abilities (KSAs)
  • Education: Bachelor's degree required, with a specialization in psychology, behavioral health, social work, special education, counseling, rehabilitation, nursing or closely related field preferred. In lieu of a Bachelor's degree, 3 - 5 years of directly relevant experience in a healthcare setting, with 6 months case management experience may be considered. (see Experience below)
  • License/Certifications: Eligibility for LSW licensure preferred. Required to travel off-site and be comfortable in unconventional settings or situations. A valid Driver's License and reliable vehicle required.
  • Experience:
  • With a Bachelors degree: 2 years' experience in a community health setting, to include 1 year of case management experience.
  • In lieu of Bachelors degree: 3 -5 years of directly relevant experience in a healthcare setting, to include 6 months of case management experience. Relevant experience provides advanced knowledge of community outreach resources, federal programs and skills in assisting clients to make informed decisions about their health and health care.
  • All Candidates: Experience Collaborating with State and Federal Programs (such as Department of Health and Human Services, Social Security Administration, Center for Medicare and Medicaid Services to name a few), community agencies, municipal and public programs.
  • Experience demonstrating a proven ability to ensure a wide variety of other disciplines or community resources work cooperatively in order to maximize the health and well-being of our clients.
  • Advanced level of analytical skills and demonstration of good judgement in assessing and planning for the needs of clients
  • Ability to work effectively without supervision, at times remotely; able to troubleshoot and solve problems independently.
  • Excellent computer skills are required; Microsoft Word, Excel, and use of electronic consumer recording software. The ability to operate software systems that support program activities, work in databases and extract statistical reports.
  • Comfort and competency developing and presenting information to outside groups is desirable.
  • Strong client/patient care, teamwork, resource knowledge and advocacy skills are essential.
  • Advanced interpersonal skills are necessary to effectively, compassionately and cordially interact with varying audiences, including clients, foreign language interpreters, health care providers and billing associates.
  • Excellent written, verbal and listening skills to explain policies and procedures on complex and life-changing topics to applicants, enrollees, providers and the public.
  • Excellent mathematical and computation skills are required to prepare reports, perform calculations, and complete forms.
  • Excellent organizational skills and attention to detail.
