Job Description:
  • PURPOSE OF JOB : Prepares and scans patient record documents into the electronic medical record consistent with Medical Staff, Administrative, and regulatory requirements timely and accurately to assure availability to authorized users for patient care, patient safety and meet Meaningful Use requirements. Provides "front-line" services to all internal and external customers that visit the HIS Department. Releases medical information assuring patient privacy and confidentialiy according to hospital and outside regulatory agency requirements, i.e., HIPAA, Joint Commission, PA Department of Health. Customers include physicians, patients, funeral home directors, and law enforcement personnel, attorneys, nursing homes, other hospitals and Associates from other departments.
  • ESSENTIAL FUNCTIONS :
1. Prepares and scans patient records into the hospital information system.

2. Assigns form identification (Form ID) to Discharge Instructions,

Advance Directives, EMS reports, inpatient and outpatient medical records.

3. Maintains a 98% accuracy standard of scanned documents.

4. Acts as first line of response to customers for patient information, either by phone or face-to-

face, in a fast-paced environment that requires problem-solving and good customer skills.

5. Assures completion of Death Certificates and required documentaton prior to releasing a body

to funeral homes or coroner.

6. Demonstrates a working knowledge of Department equipment.

7. Performs other HIS-related duties as required.

Job Qualifications:
QUALIFICATIONS :
  • Education: High School Graduate or equivalent; RHIT/RHIA credential or equivalent preferred
  • Experience: Minimum of 1 year office/hospital experience
  • Other Skills:
1. Skill in using computers

2. Skill in using multi-line phone system, including transferring calls and paging

3. Resourcefulness, good judgement; self-sufficient.

4. Good organizational skills are required to prioritize and accomplish daily tasks.

5.Medical terminology preferred.
