The Medication History Pharmacy Tech reports to the Assistant Director of Pharmacy and is responsible for accurately collecting and entering current patient medications into the hospital's information system for utilization by the physicians and providers responsible for admitting and discharging the patient from the hospital. The primary function includes patient interviews and review of internal and external systems with relevant information. The Medication History Pharmacy Tech is responsible for relaying any outstanding transition of care issues to the lead pharmacist and/or physician. The Medication History Pharmacy Tech will also perform limited medication management and/or distribution activities under the supervision of a pharmacist. The Medication History Pharmacy Tech will perform assigned responsibilities according to department policies, procedures, standards and regulations within stated time frames.

Qualifications

Qualifications: Experience: 1+ years of Pharmacy Technician experience. Prior experience with medication history/reconciliation preferred. Retail pharmacy experience preferred.

Education & Training: HS Diploma or equivalent required; Licensed Practical Nurse or Bachelor's degree preferred.

License, Certification & Registration : Certified Pharmacy Technician (CPhT); if LPN CPhT required within 1 year of hire.

Other Requirements: Excellent written and verbal skills; Proficient with basic PC skills; Good knowledge of medications, strengths available, indications.
