Memory Care Program Coordinator

Shady Nook Nursing and Rehab is seeking a Memory Care Program Coordinator for our skilled nursing home facility in Lawrenceburg, IN!

The primary purpose of the Memory Care Program Coordinator is to plan, organize, develop, and direct the overall operation of the Memory Care Program under current federal, state, and local standards, guidelines, and regulations, our established policies and procedures, and as may be directed by the Administrator.

Responsibilities:

• Plans and coordinates a therapeutic program schedule, which meets the resident's spiritual, social, emotional, physical, and intellectual needs.
• Monitors daily functioning of the unit to ensure continuity of and appropriate changes.
• Develops and maintains a cooperative relationship with other department directors. Informs and consults with staff regarding the program and integrates the program with other services.
• Assess the educational needs of staff regarding program and dementia-specific knowledge and work with HR to ensure appropriate education is provided.
• Provides program continuation for new staff unit members.
• Maintains accurate and timely documentation that complies with federal/state regulations and facility policies.
• Serves as a role model for staff regarding the care of dementia residents.
• Assists with the adjustment of the resident and family to the facility.

Qualifications:

• Bachelor's degree from an approved school of Social Work, RN, or LPN educated, with experience in dementia.
