Job Description

The Mental Health Service Corps (MHSC) is a workforce development program of ThriveNYC that aims to build a diverse generation of mental health clinicians, equipped to integrate behavioral health into a variety of settings. As a part of MHSC, early-career clinicians are placed throughout the NYC Health + Hospitals system with the goal of enhancing behavioral health delivery through existing and innovative models of care. To ensure a diverse learning experience, clinicians will have the opportunity to practice across multiple clinical settings. Additionally, Corps Members will be trained in evidence-based practices under the supervision of licensed clinicians in order to screen, assess, and treat patients in a culturally responsive manner.

Within the Office of Population Health at NYC Health + Hospitals, and under the Director, Collaborative Care, the Collaborative Care Coach will provide field support for Central Office's Collaborative Care initiatives to integrate primary care and behavioral health and improve chronic illness outcomes for patients in Health + Hospitals primary care clinics, with particular focus on depression and including other conditions such as substance use disorders and anxiety.

Principal Duties and Responsibilities will include:
  • Support implementation of the evidence-based Collaborative Care model, integrating behavioral health into primary care practice for adult and pediatric populations
  • Formulate and develop methods for monitoring and evaluating implementation and maintenance of Collaborative Care standards to determine if the program is meeting the needs, objectives and goals of the model
  • Provides guidance to primary care practices in meeting NYS Office of Mental Health and CMS requirements for quality and reimbursement via coaching and feedback on the implementation of the Collaborative Care model.
  • Support the expansion of the Collaborative Care model to new populations (e.g. maternal health, adolescent) and conditions (e.g. anxiety, substance use disorders), beyond adult depression.
  • Conduct field assessments within facilities to identify areas of strength and areas in need of development around documentation, measures and outcomes, workflows, and organizational capacity.
  • Provide guidance and motivation to promote teamwork and achieve program goals through a systems approach to Collaborative Care. Build partnerships and supportive relationships, facilitate communication and information sharing across teams, problem-solve with stakeholders to address barriers and gain commitment and collaboration to achieve success.
  • Guide facility teams in workflow optimization and quality improvement activities to assure attainment of quality care and meet Collaborative Care standards.
  • Use clinical teaching/coaching to support Collaborative Care nurses and social workers in direct clinical care delivery
  • As needed, onboard and orient new Collaborative Care nurses and social workers to the model
  • Coach Collaborative Care RNs and social workers in clinical documentation workflows in EHR and/or registry to support Collaborative Care program requirements
  • Provides guidance to facility staff (RNs, Social Workers, Population Health Managers and other team members) in the use of a registry and other population Health tools (e.g. dashboard) to drive quality patient care.
  • Use Population Health data to drive quality and process improvement. Translate data for partners delivering direct clinical care at the facilities.
  • May participate in and plan in-service trainings, workshops and lunch & learn activities for clinic staff on Collaborative Care and related topics
  • Advises the Director, Collaborative Care, on development of Collaborative Care staff, implementation of program goals and makes recommendations for improvements in the design of Collaborative Care program
  • Strong interpersonal, communication, and group facilitation skills are essential.
  • Other related duties as directed

Positions may be assigned to different NYC Health + Hospitals facilities throughout the five boroughs of NYC. Shifts/tours may vary as well.

Minimum Qualifications
1. A Baccalaureate Degree from an accredited college or university in Health Administration, Public Administration or approved related program; and,
2. Six years of full-time, paid experience of progressively responsible level in hospital administration with particular emphasis on development and evaluation of health care delivery services; or,
3. A Master's Degree from an accredited college or university in Hospital or Health Administration, Health Care Planning, Public Health, Administrative Medicine, Business Administration, Public Administration or approved related program; and,
4. Five years of full-time, paid experience as indicated in (2); or,
5. A satisfactory equivalent combination of education, training and experience.
