Description: Job Summary:

Position provides case management and short term, solution based counseling to high risk children and their families who have significant psycho-social barriers to receiving health care and maintaining optimum health. The social worker collaborates with a wide range of Kaiser Permanente health care team members and community partners.

Essential Responsibilities:

  • Provides short term case management to high risk, medically, and socially complex pediatric patients and their families served by special need plans which will result in improved compliance and a decreased risk of readmission to the hospital and or the Emergency Department.

  • Coordinates care plan developed with MD, patient/family, and appropriate community partners to promote understanding and adherence.

  • Provides single point of contact to schools serving these patients to assist with coordination of care and education of school and classroom personnel to ensure medical compliance.

  • Assist family in identifying and accessing local, state, and national public and private resources which further the child/family well being.

  • Minimum one (1) year of recent pediatric case management or counseling experience

  • This job requires credentials from multiple states. Credentials from the primary work state are required before hire. Additional Credentials from the secondary work state(s) are required post hire.
  • Clinical Social Work Associate Certificate (Oregon) within 6 months of hire OR Licensed Clinical Social Worker (Oregon) within 6 months of hire OR Masters Social Worker License (Oregon) within 6 months of hire
  • Certified Social Worker Associate License (Washington) within 6 months of hire OR Licensed Advanced Social Worker License (Washington) within 6 months of hire OR Licensed Independent Clinical Social Worker (Washington) within 6 months of hire OR Licensed Social Worker Associate-Independent Clinical (Washington) within 6 months of hire
  • National Provider Identifier required at hire
  • Basic Life Support within 3 months of hire
Additional Requirements:

  • Ability to assess and prioritize work independently.
  • Strong knowledge of Oregon and/or Washingtons Medicaid system along with knowledge of other local and national resources for children and families.
  • Expert ability to communicate and collaborate with a wide variety of people including those from other cultures.
  • Strong clinical skills with a focus on child development and family systems.
Preferred Qualifications:

  • Minimum four (4) years of recent pediatric case management experience (age ranges 0-18) in a clinical setting.
  • Highly creative problem solver.
  • Demonstrated ability to work effectively with community partners, e.g., schools, Child Protective Services, and public/private service organizations.
  • Proficient in motivational interviewing.

Pay Range: $40.73 - $59.26 / year The ranges posted above reflect the location in the job posting. The salary range may vary if you reside in a different location or state than the location posted.
Travel: Yes, 50 % of the Time
