Summary:

Provides and documents assistance to patients with activities of daily living and maintenance of a safe and clean environment, under the direction and supervision of a registered nurse.

Responsibilities:

Essential Functions •Performs personal care (activities of daily living): Bathing (assist, bed bath, tub bath, shower, sitz bath), mouth care, skin care, hair care, nail care, bedmaking (modified), dressing and undressing. •Uses appropriate body mechanics: Applies proper technique when assisting patients with turningand positioning, transfers (chair and stretcher), use of lifts, assistance with ambulation, and range of motion exercises. •Assists with patient's nutritional needs: Prepares patients for meal time, assists and/or feeds patients, measures and records intake and output, collaborates with the nurse regarding patient's increased or restricted fluid needs. •Assists with patient's elimination needs: Bedpan/urinal, bowel/bladder retraining, collect/test specimens, perineal/catheter care, applies condom caths, enemas (non-medicated), empties drainage devices from body cavities/wounds, and measures and records output. •Adheres to safety measures: Side rails/call bells, mitts and restraints, Basic Life Support including AED, CPR, Anti-Choking (Heimlich Maneuver), infection control (handwashing, isolation technique, universal precautions). Utilizes safety devices and good body mechanics to avoid injury to patient or self. Cleans equipment according to policy and returns it to appropriate area and unit. Maintains cleanliness in patient room, utility room, medication room, and desk area. Keeps hallways clear of equipment. •Special procedures: Obtains and records vital signs - temperature (oral, rectal, axillary), pulse (radial, apical), respirations, blood pressure, pulse oximetry (O2 saturation), finger stick blood sugars, height, and weight (stand-up scales/bed scales). Application of heat/cold therapy. Prevention and care for decubitus ulcers. Surgical skin preps and scrubs. Applies and removes ace bandages, TEDs and binders, and EKG monitor leads. Provides postmortem care. Assists patient with appropriate cough/deep breathing technique. •CNA II specific special procedures: Provides IV site care/dressing changes and discontinues peripheral IV catheters. Performs oropharyngeal and nasopharyngeal suction. Simple oxygen set up and flow rate monitoring (nasal cannula only). Performs urinary catheterization insertion and removal. Clamps feeding tubes and removes oral/nasogastric feeding tubes. Performs clean dressing changes and sterile dressing changes (wound over 48 hours old). Ostomy and tracheostomy care. Break up and removal of fecal impaction. •Assists in admission, discharge, and transfer process: Assumes responsibility for gathering and labeling patient belongings for transfer or discharge and accompanies patient when indicated. Orients patient/family in use of call bell, lights, phone, emergency call bell, television, and information packet, including hospital policies. Transports patients to other units/departments or to discharge area as needed. •Notifies nurse of changes in patient's condition or behavior, results of treatments, accidents, incidents, errors, and all abnormal values of patient data (such as vital signs and blood glucose). •Teamwork: Develops and maintains positive relationships with team members/internal customers. Consistently demonstrates a willingness to adjust activities to accommodate the needs of team members/internal customers. Resolves conflicts independently, as appropriate. Accepts responsibility for actions and responds positively upon feedback from others. •Restocks routine medication administration related supplies for nurse and may have access to noncontrolled patient medications; is required to ensure unattended storage device within a patient's room is secure and locked at all times to prevent unauthorized access; is required to alert pharmacy or nursing personnel if unsecured medications are found.

Experience: 1 Year of Experience Preferred

Refer to the Life Support Training Policy NH-HR-3096.

Licensure/Certification Type (ex NP, RN, CMA): Completion of NA I and NA II programs required.

NA registry as NA I in good standing. Current NA II certification with state required

NC state approved curricula for restrictive interventions (facility specific) within 3 months of hire for Emergency Departments and Behavioral Health units required

Additional Skills/Requirements (required) •BON Health Care Provider. Medication Safety - AME

Additional Skills/Requirements (preferred) •Basic computer skills for documentation
