Job Description

The Department of Psychiatry in the School of Medicine is seeking a Nurse Case Manager for the Bridge Program. Bridge is a school-based psychiatry program that provides psychiatric evaluations and treatment for school-aged children in Boone County who are experiencing emotional or behavioral health issues. This program makes psychiatric treatment more accessible and timelier, helps reduce school crises, and gets children back on track to learn, socialize and feel better.

This position reports to the Manager of the Bridge program and works to provides necessary services to achieve the program goals and outcomes serving children. This position collaborates with the Child Psychiatrists working in the program and other staff on the project.

This is a fully grant funded position and program. The funding requires a Case Manager have an RN license.

A successful Nurse Case Manager candidate will excel at the following primary responsibilities:

- Consult with schools and other community providers within Boone County for student referrals;
- Coordinate with outreach counselors for completion of program forms and releases;
- Schedule appointments and attend school-based clinics with the program's psychiatrists;
- Maintain regular contact to and from parents/caregivers, school staff, and other providers of care for each student-participant;
- Attend meetings with school staff and other school-based providers to collaborate, refer, and share students' progress;
- Maintain data collection and assist in interim and end-of-year reporting.

Shift

8am - 5pm, Monday through Friday.

Minimum Qualifications

• Three (3) years of clinical experience, which provides knowledge, and understanding of medical terminology, patient diagnoses, treatment protocols and disease process.

• One of the following:

-Master of Social Work degree, or Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) licensure by the Missouri State Committee for Social Workers board, or

-Missouri Board of Nursing RN or Nurse Licensure Compact multi-state RN. When primary state of residency changes, compact state RNs must apply under new state of residency within thirty (30) days, or

-Occupational Therapist (OT) licensure by the Missouri Board of Occupational Therapy, or

-Speech Language Pathologist (SLP) licensure by the Missouri Commission for Speech-Language Pathologists, and Certificate of Clinical Competency (CCC) certification from American Speech-Language-Hearing Association (ASHA), or

-Physical Therapist (PT) licensure by the Missouri Board of Registration for the Healing Arts or PT Compact multi-state priveilege. When primary state of residency changes, compact state PTs must apply under new state of residency within thiry (30) days.

Preferred Qualifications

Daytime travel is frequent and required within Boone County, as visits to schools are necessary. Reliable transportation is essential. Travel costs are reimbursed.
