Job Summary
The Nurse Manager, Hospital at Home (NMHH), will be spearheading Harris Health System's vision to lead the way in delivering care outside of the boundaries of a traditional hospital. This role will be accountable for all operational, human resource, quality, and financial management aspects of the Hospital at Home program. The NMHH will work collaboratively with the Medical Director, Hospital at Home, in the implementation of plans and policies in adherence to standards of the program. The position will be accountable for planning, implementing, evaluating and communicating all phases of patient care within the program. A project management, process improvement, and organizational skillset will be vital to this role, as the NMHH will be creating and monitoring all operational workflows for the program.
The NMHH also provides direct and high quality patient care to the patient population within the framework of the Hospital at Home program. This role will be responsible for all clinical functions and will also provide direct, bedside care in patients' homes, that will include, but are not limited to, the following responsibilities: observe and record patient behavior, perform physical examinations and diagnostic tests, counsel and educate patients and their families on treatment plans, administer medication, change wound dressings and care for other treatment options, and consult with supervisors and doctors to decide on the best treatment plan for patients. The NMHH is responsible for facilitating an environment that supports professional nursing practice and empowers nurses to provide safe, effective, compassionate, and efficient, patient-focused nursing care.
Minimum Qualifications
Degree:
a) Masters: Master of Science in Nursing from an Accredited Nursing Program, Healthcare Administration, Business Administration or Public Administration required
Licenses & Certification:
a) Registered Nursing: State of TX
b) CCM or ACM or other clinical, nursing-related certification : Specialty Nursing Certification
c) Texas Driver License
d) Basic Life Support: From an American Heart Association or Red Cross Program
Work Experience:
a) 5 Years of Experience: Complex healthcare environment including the acute care setting; case management experience also preferred
Management Experience:
a) 1 Year of Experience: Healthcare Leadership or Charge Nurse experience
Communication Skills:
a) Above average Verbal Communication (Heavy Public Contact)
b) Exceptional Verbal (Public Speaking)
c) Writing/ Correspondence
d) Writing/ Reports
Language:
