The role you'll contribute:

The Nurse Navigator coordinates all services required to provide multidisciplinary, quality assured,

appropriate case selection and cost-effective care to wellness and prevention patients. The Nurse

Navigator serves as a resource for patients, physicians and others serving in the role of educator, quality improvement and consultant to patients referred. Makes initial contact with the patient/family to establish a professional relationship and understanding of the diagnosis, consultation, treatment and follow-up regimen. The nurse assesses the patient's status and works with physicians to determine appropriate plan for patient and assess appropriateness per regulatory and vendor guidelines. Develops, manages and redesigns, as needed, the process for patients identified through the program. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

• Develops, plans and implements evidenced-based measures that increases the quality of care for the high risk hereditary breast & ovarian cancer and High risk colon cancer patient care and ensures compliance with all regulatory guidelines.

• Navigate the High Risk: Provide additional support and resources in the form of care navigation to guide patients through high risk care pathways & ensure ther are no gaps in care

• Assist in the development of Care Pathways and Algorithums that are based widely on accepted evidence-based practice guidelines form industry leading/recognized orgnaizations to personalized care based on risk

• Supports and assists with data collection for process and quality improvement while understanding the department performance trends and established standards. Works collaboratively with the Genomic and Personalized Health Team

• Keeps Medical and Administrative Manager/Directors informed and seeks assistance and direction from appropriate individuals when confronted with unfamiliar procedures and techniques before implementing them to ensure delivery of safe and effective patient care.

• Coordinates patient education and care: serves as an ongoing resource for patients and families during their diagnosis and treatment period; communicates information regarding patient status and needs; collaborates with other providers regarding how to best meet needs and sequence care; assesses patients' learning style and health literacy in order to provide education on the appropriate patient level.

• Connect with Identified patients through the risk assessment survey and assist with additional steps of testing; may include genetic counseling, genetic testing, breast MRI scans and/or ultrasounds

• Based on findings from the genetic testing, additional family members could be tested and assessed as well.

• Collect program metrics as defined by program

• Develop connections and relationships within the organization to ensure timely patient care.

• Provides telephone and face-to-face consultation with patients to answer pertinent questions throughout their experience; provides educational information to patient/family and reviews information with patient. Continues to communicates with patient and family to clarify any outstanding concerns to ensure patient understands follow-up.

• Communicates and collaborates with multidisciplinary team to ensure high-quality patient care.

• Acts as a liaison to physicians' offices regarding proper orders, medical records, consents and any other required documentation/criteria ensuring they are received at the time of scheduling a patient for a procedure.

• Maintains a professional relationship with all customers.

• Completes assigned work comprehensively and in a timely manner.

• Facilitates scheduling with coordinators, supervisors, physicians and patients.

Qualifications
The expertise and experiences you'll need to succeed:

Minimum qualifications:

• Effective verbal and written communication skills

• Ability to read, write and explain moderately complex information to hospital and clinical staff with a great deal of attention to detail

• Ability to communicate effectively in a supportive manner to patients, family, staff and physicians

• Ability to explain information to patients and families in a way the patient understands, and/or explain departmental or hospital procedures to employees, visitors and patients

• Ability to observe assess, guide and evaluate others

• Ability to communicate timely and in an open and honest manner

• Ability to collect data, interpret findings, set priorities and carry out established plansFlexibility in prioritization; ability to multitask; critical thinking/problem-solving skills

• Self-motivated with ability to work independently and in a group setting

• Strong organizational skills

• Bachelor of Science degree in Nursing

• Three years of experience in oncology or navigator role exerience

• Current license with Florida State Board of Nursing as a registered professional nurse

• Current valid BLS and ACLS

Preferred qualifications:

• Master's degree in Nursing, Business Management or Healthcare Management

• Previous experience with projects or program start-up

• Speciality certification as defined by cost center
  • Six Sigma (or equivalent)
