SUMMARY
The Heart Failur Nurse Navigator improves patient preparedness for treatment through education and psychosocial support while also facilitating interaction between patients, their physicians, and referrals as appropriate. The Heart Failure Nurse Navigator assists in the development of appropriate educational materials and program monitoring to promote coordination of care and patient satisfaction. The Heart Failure Nurse Navigator assists the clinical and leadership team with program development, expansion and successful achievement of programmatic goals. The Heart Failure Nurse Navigator will communicate effectively with patient ensuring seamless plan of care and understanding of follow up needs.
QUALIFICATIONS
Performs additional duties as assigned Establishes Post-discharge Quality Assurance
Communicates professionally and effectively in written and verbal form Supports Stroke Program's Joint Commision compliance
Provides support to patient and care partners through education, guidance, and direction

Required:
- 3 or more years job related experience, including experience in Neurosciences
- Bachelor's Degree in Nursing
- Registered Nurse in State of Georgia Basic Life Support (BLS)
- Preferred: SCRN
