Purpose of Position
The Nurse Practitioner is licensed to practice medicine in the Commonwealth of Massachusetts and is appointed by the Administrator on the approval of the Board of Trustees. The Nurse Practitioner has designated administrative responsibility to provide policy formulation, clinical oversight and quality improvement in ensuring regulatory compliance. Through consultation, approval and guidance in the development and implementation of specific medical or rehabilitative protocols, the Nurse Practitioner ensures the clinical integrity of resident care. In collaboration with others, s/he provides a level of care and an atmosphere that is warm, friendly, respectful and compassionate, as well as safe and competent.

Description

1. Must be courteous to patients, visitors, physicians, volunteers and other employees of the organization.
2. Provides the standards, coordination, surveillance and planning of medical care in our facility.
3. Reviews and evaluates resident/patient care policies and procedures.
4. Updates the physician on the progress of the patient.
5. Documents in the medical record, as applicable, to maintain compliance.
6. Assures that resident/patients on an individual case basis receive adequate medical care.(Reviews individual cases with the professional staff.)
7. Monitors the quality and appropriateness of medical care, as an integral part of a total care program and the quality assurance program.
8. Advises on Admission, Discharge, Transfer policies.
9. Participates in and advises in-service training programs for staff development.
10. Assists with the QA and QI process as needed or deemed necessary for continued quality improvement.
11. Maintains compliance with JCAHO and OBRA Regulations.
12. Has been educated in, and adheres to, Universal Precaution and Infection Control Policies.
13. Completes a review and evaluation of patients on an annual basis.
14. This review includes history and physical, medical care plan review, current lab work, care planning needs, with summarization in the medical record.
15. Has a good working relationship with the Hospital Staff.
16. Serves on the following committees: Quality Assurance, Infection Control, Pharmacy and Therapeutics, Patient Care Policies, Safety, and as ex officio on all other committees.
17. Serves as liaison between Administration and Supervisory Physicians to monitor the quality and appropriateness of medical care as an integral part of the total care program.
18. Provides continuous services to the Hospital during the term of this agreement.
19. Must wear facility provided identification badge.
20. Other work-related duties may be assigned.
21. Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers.
22. Challenges current working practices; identifies process improvement opportunities and presents recommendations and solutions to management. Engages and commits to the organization's culture of continuous improvement by actively participating, supporting, and promoting CCHC Pillars of Excellence.

Qualifications

• Master's Degree in nursing.
• Must be a licensed Nurse Practitioner in the Commonwealth of Massachusetts
• Demonstrates advocacy skills on behalf of residents/patients and staff.
• Ability to maintain constructive working relationships with people from diverse backgrounds.
• Demonstrates effective oral and written communication skills.
• Ability to identify, define and solve problems, utilizing the skills of available resources both within and outside of Falmouth Hospital.
• Is compassionate, empathetic and understanding of the individual needs of residents/patients.
