GENERAL SUMMARY/ OVERVIEW STATEMENT: Summarize the nature and level of work performed.

Working under the general direction of the Section Head for Interventional Cardiology and a collaborating physician, the advanced practice clinician (Physician Assistant or Nurse Practitioner) provides direct care to patients in a high volume outpatient clinical service. S/he develops, implements, and evaluates a program of specialized health care for a select patient population with cardiovascular disease. S/he promotes a collaborative relationship among various health care professionals toward continuity of patient care and efficient use of resources. S/he serves as a role model for professional medical practice in the management of patients with cardiovascular disease.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Outpatient APP Role:

• The APP works collaboratively with the Interventional Cardiology Program Leadership, Physicians, RNs and PSCs as a provider in the outpatient clinic. The APP practices in accord with practitioner guidelines per state and hospital regulation

o Manages follow up patients in collaboration with the Collaborating MD enhancing continuity of care.

o Ensures timely appointments and facilitates access to care for patient

o Performs and records the patient history and physical exam

o Utilizes knowledge & training to identify, prevent or solve complex and/or recurring patient care problems.

o Orders appropriate procedures and testing particular to the select patient population according to guidelines.

o Performs medically delegated aspects of patient care according to practice guidelines.

o Collaborates with health care professionals within other cardiology sub specialties and outside of cardiology to facilitate a comprehensive plan of care and coordinate resources.

o Maintains and promotes relationships with health care providers, both within the institution and outside of it.

o Serves as resource to health care professionals in area(s) of expertise.

o Monitors patient outcomes in accordance with defined standards of patient care.

Clinical Responsibilities:

• The APP will perform independent patient visits both planned follow up care and urgent care as well

o This will include Virtual Telemedicine visits in addition to in person follow up and urgent visits.

o The APP will be accountable for ~1200 patient visits per year.

o The schedule will include 5-6 4 hour sessions with possible additional follow up or urgent patients as needed to meet the needs of patients and the clinic.

o The equivalent of 2 days will be allotted for administrative time including (but not limited to) in basket follow up, returning patient phone calls, completing documentation, Rx refills.

o The APP participates in cross coverage of clinics and in basket when colleagues are away.

Education and Professional Growth

Participates in the development of formal and informal educational programs for health care professional

• Responsible for own professional growth and maintenance of required credentials.

• Maintains the standards and educational requirements specific to the position and licensure.

• Takes responsibilities for leadership and participation on committees and task forces of the health care system.

• Maintains and updates clinical knowledge and skills based on current health care practice/research.

Qualifications

Cardiology Experience preferred

• Current Massachusetts licensure as a registered nurse in the expanded role (NP) or Physician Assistant (PA)

• Current Federal & Massachusetts licensures to dispense Controlled Substance

• Certification in advanced practice nursing or physician assistant

• Certification in Basic Life Support

• Certification in Advanced Cardiac Life Support

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED: (MUST be realistic, measurable, objective, and related to the essential functions of the job.)

• Fluent in verbal and written language skill

• Demonstrated competence in providing cardiac care

• Effective communicator with the ability to transfer knowledge (orally and in writing)

• Computer skills to support administrative and clinical tasks, to include EHR & Microsoft Office application

WORKING CONDITIONS: Describe the conditions in which the work is performed.

Work is performed in both a physician medical office and a licensed hospital setting. The work may require the lifting of heavy objects and patients, as well as exposure to potentially infective material. Work requires contact with needles and other sharp instruments, monitoring devices, and other state-of-the-art equipment. Walking throughout the ambulatory space and hospital facilities as well as prolonged standing can be expected.

Maintains eligible enrollment with contracted health insurance provider
