InterCommunity HealthCare seeks a compassionate, dedicated and skilled Advanced Practice Registered Nurse (APRN) specializing in Family Medicine to join our team of elite providers. The APRN will be committed to delivering high-quality, "whole person" healthcare services to our diverse patient population. As an integral part of our team, you will be able to provide comprehensive care to complex patients from all walks of life and across all age groups, including disease-based treatments and care for individuals with substance use disorders.

Essential Duties & Responsibilities:
The APRN will be committed to providing integrated healthcare focusing on primary and addiction health across all age groups. The Family Medicine Provider will deliver comprehensive care in accordance with InterCommunity HealthCare policies and federal and state regulations, including the treatment of disease-based conditions and substance use disorders.

Key Responsibilities:
Obtain health histories and conduct thorough physical examinations.
Diagnose and develop individualized treatment plans, including counseling.
Ensure timely closure and signature of patient charts/encounters, preferably by the end of the workday but no later than 24 hours.
Practice with Disease Prevention in mind, ensuring all care gaps are closed for our patients.
Maintain continuity of care and review lab/procedure results, such as labs, imaging, ECGs, etc.
Adhere to established departmental and medical center policies, continuous quality improvement objectives, and safety, environmental, and infection control standards.
Attend all agency or department meetings.

Additional Responsibilities:
Demonstrate patience and empathy in providing care to patients with psychiatric disorders and a history of substance use or current substance use.
Participates in specified health promotion, education, and/or prevention programs as needed, such as diabetes collaborative and health fairs; attends and contributes to medical center meetings, departmental meetings, and other medical center functions as required by the Center.
Assists in quality improvement initiatives.
Performs other related work as required.

Required Skills & Experience

Master's or Doctorate Degree in Nursing from an accredited University.
Active licensure with the State of Connecticut as an Advanced Practice Registered Nurse (APRN).
Current DEA (Drug Enforcement Administration) registration.
Experience in treating and managing patients with acute and chronic conditions.
Proficiency in electronic health record (EHR) systems and medical documentation.

Nice to Have Skills & Experience

Bilingual in Spanish
PNP or FNP certification
