Job Description

SUMMARY OF DUTIES AND RESPONSIBILITIES
  1. Identifies and discriminates between physical and psychological signs symptoms relating to the health status of individuals and families through health and development history taking and physical diagnosis consistent with established nursing practices.
  2. Consistent with established nursing practice differentiates between normal and abnormal findings in the screening process to detect changes in the health status that require the physician's intervention.
  3. Evaluates and interprets patient data in order to plan and implement appropriate nursing intervention.
  4. Consults with the physician for evaluation of findings of the health screening in order to make prospective decisions about treatment.
  5. Manages the care of selected patients within protocol mutually agreed upon by nursing and medical personnel.
  6. Records and maintains a system of patient progress notes to plan program management and therapy appropriate to patient's needs.
  7. Collaborates with other members of the interdisciplinary team in order to determine, evaluate and attain health goals.
  8. Determines possible alternative settings for providing health care (i.e. home or institution) and initiates referrals.
  9. Conducts and participates in community clinics for case findings.
  10. Provides to patients and families information relating to the implication of health status, treatment and progress. Counsels and guides patients and families in order to develop favorable attitudes toward health care and maintenance and to encourage practices that promote health and well-being and reduce anxiety, stress and disability.
  11. Provides teaching, guidance and direction to nursing personnel in rendering health care and assists in directing and coordinating nursing activities.
  12. Prescribes and makes adjustment on medications within established protocols.
  13. Attends professional meetings, workshops and conferences to enrich knowledge and to improve clinical skills.
  14. Participates in planning preceptors and educational experiences for medical and nursing students, other related health care professionals.
  15. Serves on professional, nursing and hospital wide committees.


Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
2. Current certificate as a Nurse Practitioner issued by the NYSED; and
3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Department Preferences

EDUCATIONAL LEVEL:
  1. A valid New York State license and current registration to practice as a Registered Professional Nurse and
  2. Current certification as a Nurse Practitioner issued by the New York State Department of Education and
  3. A current certificate issued by the State Education Department to prescribe medication and
  4. Two years satisfactory experience as a Nurse Practitioner, or two years satisfactory experience as a Registered Professional Nurse, eighteen months of which must be as a Nurse Practitioner
