Job Description

Purpose:

Nurse Practitioners may participate in the evaluation, assessment and triage of patients in collaboration with Attending Physicians participating in the care of patients focusing on navigation of cancer screening and follow up.

Specific Duties and Responsibilities:

The Nurse Practitioner is charged with and responsible for:
  1. Launch educational and patient engagement initiative to maximize the impact of breast and lung screenings.
  2. Counsel and educate patients on work-up and order appropriate imaging and referrals.
  3. Connect patients to tobacco cessation.
  4. Prevent loss to follow-up throughout multistep screening and workup processes.
  5. Develop a tracking system for coordinating with multiple services to ensure appropriate work-up and follow-up.
  6. Maximize impact and ensure financial and administrative sustainability
  7. Develop quality assurance/performance improvement framework that integrates a Central Office-based program management team with leaders and key stakeholders across the system.
  8. Prepare presentations for Quality departmental/central office meetings
  9. Perform History & Physical examinations
  10. Order routine diagnostic testing, e.g., laboratory, radiologic, etc.
  11. Ongoing medical documentation in the medical record, e.g., progress note, etc.
  12. Refer for appropriate consultation (s) and make appointments as appropriate
  13. Order medications and write prescriptions subject to Section 6902 (3) (b) of the New York State Education Law.
  14. Liaison for central office and system sites
  15. Maintain continuing medical education to maintain privileges
  16. Other duties as required


Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
2. Current certificate as a Nurse Practitioner issued by the NYSED; and
3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
