The Opportunity

The role of the Nurse Practitioner, Virtual Resource Center is a critical position for the Wentworth Health Partners team. This position is responsible for supporting the care of our primary pare patients. We are seeking a dedicated and experienced Nurse Practitioner to see patients in-person and virtually, remotely manage Epic in-baskets and coordinate care for patients of all ages. This role demands clinical expertise, excellent communication skills, and a deep understanding of the Epic system's nuances. The chosen candidate will serve as a pivotal point of communication and care among patients, providers, and other healthcare professionals.

Additional duties and responsibilities include:

  • Caring for primary care patients both in-person and virtually
  • Responding to patient portal messages in a timely manner
  • Assisting primary care clinicians with Epic In-Basket management
  • Evaluating incoming/routed patient questions and, when urgent, create a telemedicine visit
  • Diagnosing and treating patients via telemedicine, including recommending suitable treatment plans, and considering cost-effective treatment modalities
  • Collaborating with clinicians in managing acute and long-term medical needs of patients
  • Maintaining continuity of care between physician visits
  • Maintaining and reviewing patient records, charts, lab results, and other pertinent information
  • Providing medical and wellness education
  • Prescribing medication, including support of refill pools, in accordance with state statutes and professional practice guidelines
  • Collaborating with specialty care as appropriate
  • Reviewing/managing incoming reports (e.g. lab, imaging)
  • Participating in peer review, quality assurance, provider meetings, and other clinical meetings
  • Assisting in updating/implementing protocols and principles of practice as requested


Location & Schedule

This is a full-time, 40-hour per week hybrid position. The Nurse Practitioner will work Monday-Friday, 8:00am to 5:00pm splitting the week on-site versus remotely. We are located at 67 Corporate Drive Building A, Portsmouth, NH 03801.

Education:

  • Master's Degree in Nursing required


Experience:

  • Previous Nurse Practitioner experience preferred - strong preference to those with prior Primary Care/virtual care experience


Certification:

  • Certified New Hampshire Nurse Practitioner required upon hire
