Responsibilities
Oncology Nurse Navigator
Join our dynamic team of Oncology Navigator RNs become an integral part of the team and collaborate with our board-certified dieticians psychotherapist and genetic counselors. At the Dignity Health Cancer Institute you will experience exciting breakthroughs in the field of thoracic cancer! This position will be based at Mercy San Juan Medical Center working closely alongside thoracic surgeons and physician assistants as well as outpatient cancer centers. In this unique role you will focus on day to day planning and implementation of a broad-based community care coordination patient navigation and education programs provided to patients receiving surgical medical in-patient or outpatient healthcare services in Cancer Care.
Help patients navigate the full continuum of cancer care from point of diagnosis through to survivorship or end of life by:
The Oncology Care Navigator must be passionate about excellence in oncology patient care and aspire to offer the best possible care. Proven ability to work with a team excellent communication skills ability to provide emotional support and empathy while developing innovative solutions to patient care barriers and ensuring no patient care delays.
Familiarity with oncology treatments and their side effects is essential.

- Providing care coordination education and resource referrals regarding patient needs.
- Facilitating patient care services such as; transportation translation services support groups MD referrals financial and resource referrals.
- Identifying and removing barriers to care within specific communities.
- Be the central point of contact for individual families as they navigate the complex healthcare system.
- Developing positive relationships in order to collaborate with interprofessional team members as well as with community organizations project partners and other healthcare system leaders.

Qualifications
Minimum Qualifications:
- Current CA RN licensure
- Bachelors Degree in Education Healthcare Community Health or a related field or relevant experience, or equivalent combination of education and experience.
- Minimum 2 years RN experience in Cancer Care and Treatment.
- Required training or certification in patient navigation or willing to successfully complete training in patient navigation.
Soft skills required:
- Good presentation and computer skills.
- Experience in educating and/or navigating adults one-on-one and in larger
groups. - Comfortable in dealing with death and dying.
- Ability to deal with people in extremely stressful situation
Preferred Qualifications:
- Previous navigation experience
- Masters degree
