Job Summary

The Weight Management / Bariatric Dietitian provides Medical Nutrition Therapy (MNT) to individuals for nutrition-related problems and preventive health measures as they apply to bariatric surgery and adult weight loss. Primary responsibilities include assessing and monitoring the nutritional health of weight management / bariatric patients, developing and implementing individualized nutrition care plans, providing nutrition counseling for individuals, families, and groups, and collaborating with the interdisciplinary health care team to facilitate continuity of care at home and in the community.

Duties

The Outpatient Weight Management / Bariatric Dietitian's duties include but are not limited to the following:
  1. Assesses client's dietary patterns, lifestyle, food purchasing practices and resources, weight history, and ability to comply with lifestyle changes.
  2. Works with clients to develop and implement individualized nutrition care plans in accordance with evidence-based standards of care, the client's medical therapy goals, and nutrition prescription.
  3. Assists clients in developing realistic, long-term health goals and goal-directed behaviors.
  4. Requests pertinent nutrition related labs from the client's provider as deemed necessary.
  5. Evaluates care plan effectiveness and measures progress towards outcomes of medical nutrition therapy interventions.
  6. Provides appropriate follow-up in accordance with patient's treatment goals, referring patients for community services as appropriate.
  7. Provides client with appropriate nutrient supplementation and treatment interventions as needed.
  8. Utilizes Telehealth for clients with the provided equipment if requested.
  9. Facilitates the use of a telephone interpreter (or sign language interpreter) for clients in need of language assistance.
  10. Directs and/or educates the Medical Clinic's Medical Assistant(s) and Patient Access Clerks on job duties and procedures as related to the Outpatient Dietitian. Maintains a written procedure on this as a training tool and reference for the Medical Assistant(s) and Patient Access Clerks.
  11. Document vitals in the client's electronic health record as provided by the Medical Assistant.
  12. Schedules or reschedules outpatient nutrition counseling appointments as needed.
  13. Conducts monthly Bariatric Nutrition Group Classes. This includes obtaining the patient's weight, and documenting the patient's weight as well as the Bariatric Nutrition Class session in the electronic health record.
  14. Composes and submits necessary completion letter for insurance prior approval.
  15. Schedules or reschedules client's counseling appointments as needed.
  16. Participates in departmental and hospital wide mandatory in-services and meets the mandatory annual in-service requirements in utilities management, infection control, safety and blood-borne pathogens.
  17. Other duties as assigned.
Reports to / Evaluated by:
  1. The outpatient dietitian reports to the Chief Clinical Dietitian and / or the Director of Nutrition Services.

Lifting Requirements: The light category requires that an employee be able to lift a maximum of 20 pounds and frequently lift and/or carry objects weighing up to 10 pounds.
