Essential Function
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Engage patients referred or identified for services and other as needed via telephone, email, and electronic health record messages.
  • Provide community outreach with attendance and participation in local community events to bring attention to the language or disability-focused services available at Jefferson Einstein Philadelphia
  • Provide culturally sensitive services to patients from diverse racial, cultural, and socioeconomic backgrounds.

Qualifications

  • Native-level fluency in foreign language preferred (Spanish, Russian, Arabic, or French Creole particularly preferred)
  • High School Diploma, or equivalent required
  • Medical Assistant preferred
  • Experience working with populations with disabilities preferred
  • Two years of work experience as a patient navigator or community health worker preferred
  • Previous work in local community settings or healthcare system preferred
  • Strong oral and written communication skills in English required. Strong oral and written communication skills in foreign language preferred
  • Proficient in all Microsoft Office Suite, including Outlook and Excel
  • Strong commitment to social justice and health equity
  • Assessment and problem-solving skills
  • Ability to work independently and within a multi-cultural diverse team
  • Demonstrated organizational and time management skills
  • Demonstrated ability to work effectively and provide advocacy for racially and ethnically diverse populations and communities preferred


Conditions of Employment
