Summary
  • Hours for this position are Monday- Friday 8AM - 4:30PM
  • Position Summary
    • The Outreach Specialist will be responsible for printing and maintaining lists of patients due for preventative care. Responsibilities include calling patients and scheduling necessary appointments as needed to meet quality metrics and close patient gaps. Working with specialty care departments to assist with scheduling annual visits due (i.e. colonoscopies, mammograms) and work with Practice Care Coordinator to ensure appropriate orders are in place to schedule. Update EMR as necessary for patients no longer in the practice due to death, move or changed PCP. Collaborate with Practice Manager to maintain the list of patients for panel/population management and call patients due for annual physicals, annual wellness visits, etc. Maintain the HCC gap list and schedule future appointments to meet the needs of the patients and close HCC gaps. Preview future provider schedules for quality items due and note appropriately for visit. Outreach Specialist will update patient EMRs as needed. This position will work closely with the Practice Care Coordinator to ensure patients' care gaps are addressed in a timely manner and report weekly to practice leadership on progress of patients called and outcomes.
  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Education: High school diploma, general education degree (GED) or equivalent is preferred.
    • License/Certifications: N/A
    • Experience: Experience in customer service preferred.
    • Proficiency in computer applications and databases is required
    • Detail-oriented and able to complete projects independently
    • Excellent grammar, writing and decision-making and organizational skills are essential
    • Familiarity with medical terminology is essential
