Schedule: Full-time, days

Location: 7700 S BROADWAY, Littleton, 80122

The value you'll bring to the team:

• Provides independent advanced practice nursing care including but not limited to completions of assessment, diagnosis, development of treatment plan including outcome identification, patient/family education, and implementation and evaluation of the treatment plan.
• Performs patient rounding with an interdisciplinary team and participates in case conferences.
• Performs comprehensive physical, psychological, social, spiritual, and cultural assessments through observation, interview, review of medical records and clinical data inclusive of imaging and lab results.
• Evaluates patient's response and intervenes to ensure optimal patient outcomes

Qualifications
The expertise and experiences you'll need to succeed:

• Palliative care experience desired - inpatient or outpatient
• Bachelor's degree, Nursing- Required
• Master's degree, Nursing- Required
• Basic Life Support- American Heart Association - Required
• Registered as Advanced Practice Nurse - Required
• Registered on the State of Colorado Advanced Practice Registry - Required
