The role you'll contribute:

The Palliative Care Coordinator assists in formalizing and developing care into a clinical service line for acute care patients and families. Provides initial and ongoing clinical assessment for patients with advanced and/or progressive medical conditions who are referred for palliative care consultation. Major focus includes symptom management, assistance with decision making, especially in the context of the critical care settings and nonbeneficial treatment, collaborating with the interdisciplinary team to provide and facilitate emotional, spiritual, and bereavement support, and coordination of care until death or discharge. Modifies the plan of care, as needed, in collaboration with the attending and/or consulting physicians and through the use of approved order sets. Acts as a resource for clinical issues that arise. Participates in quality improvement activities. Organizes, conducts, and supports the palliative care interdisciplinary group meetings.

• Performs initial palliative evaluation with patient/family - inclusive of health history, and psychosocial determinants.

• Assists with palliative care consultations and interventions for early initiation of chronic disease management of symptoms related to life-limiting conditions.

• Collaborates with patient's physician, other health care team members, the patient and family in developing realistic care expectations based on the patient's goals of care.

• Assists in the development and implementation of the interdisciplinary treatment plan with members of the palliative care consult team.

• Assists patients with advance care planning.

Qualifications
Minimum qualifications :
  • Associate degree in Nursing with minimum of 3 years clinical, hospice, or hospital setting.
  • State of Florida Registered Nurse license

Preferred qualifications:

• Bachelor's or Master's degree in nursing.

• Clinical nursing care experience in palliative care, oncology, hospice, geriatrics, or intensive care.

• Certified Hospice and Palliative Nurse (CHPN).

• Care Management experience in a hospital setting.
