Purpose of Position
We are seeking a compassionate and experienced Palliative Care Services Specialist to join our team. As a Palliative Care Services Specialist, you will play a critical role in providing care and support to patients with serious illnesses and their families. This is a part-time (3 days per week) position. In this role, you will be responsible for coordinating and managing the care of palliative care patients. You will collaborate with an interdisciplinary team, including physicians, nurses, social workers, and other healthcare professionals to develop and implement personalized care plans that address the physical, emotional, and spiritual needs of patients. You will also provide education and support to patients and their families, ensuring they understand their treatment options and helping them make informed decisions. Our ideal candidate has a deep understanding of palliative care, advanced care planning, and end-of-life care. You must be a skilled communicator, able to effectively engage with patients, families, and members of the healthcare team. Strong organizational skills are also essential to help manage multiple cases and ensure that all patients receive appropriate and timely care

Description
1. Provide as needed in-person consults to evaluate criteria for palliative care program
2. Conduct thorough assessments of patients' physical, emotional, and spiritual needs.
3. Develop and implement individualized care plans for palliative patients at risk for adverse health outcomes.
4. Collaborate with an interdisciplinary team to develop comprehensive care plans for palliative care patients.
5. Monitor patient progress and adjust care plans as necessary.
6. Provide ongoing education and support to patients and their families.
7. Coordinate and facilitate communication between patients, families, and the interdisciplinary team.
8. Document all care provided and maintain accurate and up-to-date records.
9. Provide education and support to patients and families about palliative care, advanced care planning, and end-of-life issues.
10. Advocate for patients' rights and ensure that their wishes and preferences are respected.
11. Utilizes organizational skills to manage time efficiently and prioritize differing patient needs and stages of health.
12. Consistently provides service excellence to all patients, family members, visitors, volunteers, and co-workers.
13. Refers to community resources as appropriate.
14. Maintains relationships with facility discharge planners & community entities to increase awareness of VNA Palliative Services
15. Serve as Palliative Services Liaison with physician's offices & CCHC affiliate partners as well as other Cape Cod Community
16. Challenges current working practices; identifies process improvement opportunities and presents recommendations and solutions to management. Engages and commits to the organization's culture of continuous improvement by actively participating, supporting, and promoting CCHC Pillars of Excellence

Qualifications
  • Masters Degree in Nursing preferred
  • Licensed Registered Nurse (RN)
  • Experience in palliative care or a related field
  • Strong knowledge of palliative care principles and practices
  • Experience in care coordination or case management
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively in a team environment
  • Strong organizational and time management skills
  • Compassionate and empathetic approach to patient care
  • Knowledge of end-of-life issues and advanced care planning strategies
  • Proficient in electronic health record systems and other relevant software applications
  • Ability to maintain confidentiality and adhere to ethical standards in patient care
  • Current CPR certification
