Overview
This Peer Support Specialist role will work with teens and families who are engaged in a project exploring innovative ways to increase access to mental health and substance use care and support. As part of a multi-disciplinary team, you will help teens and families to collaborate with community resources, schools and medical clinics to address the mental health, substance use and wellness needs of the adolescent. You will support the teen and/or their family empathically, to facilitate progress towards common goals. You will help to facilitate learning and engagement with school systems, mental health and substance use treatment and independent health and wellness activities, including the use of web-based programs and digital applications. You will work closely with a team of medical, mental health, addiction treatment and education professionals and will fulfill a critical role in elevating youth and family voice through partnership and advocacy.

Responsibilities
  • Reaches out to patients identified by SDoH screenings and referrals.
  • Provides assistance in the identification and referral to resources for the health care team and patients/families. Meets with patient to assess the need for services. Consults, educates, and directs the health care team to clarify the need for services. Determines the availability of services required. Provides patient information and support concerning available resources. Assists patient in obtaining appropriate resources. Maintains expertise in resources for addressing SDoH needs.
  • Visits patients at home or community locations as needed.
  • Updates the Community Resource Directory, utilizes RedCap database for tracking of interactions and documents in EPIC EMR.
  • Implements evidence based curriculum as needed.
  • Identifies and contributes to the development of systems and resources needed to meet the support needs of patients/families and staff/institution.
  • Assists in marketing Community Health work and Social Determinants of Health impact to providers and system.
  • Performs other duties as required or assigned.

  • Qualifications
    • Bachelor's degree in Human Service or related field preferred. High School diploma/GED with equivalent experience considered.
    • Excellent interpersonal, communication, and organizational skills required.
