Job Description

PURPOSE OF POSITION:

This position has overall responsibility for the supervision and development of staff who facilitate organization improvement and support safety across NYC Health+ Hospitals/Metropolitan. The Associate Director of Health Care Standards Level I, will develop staff competencies, assess individual performance, evaluate program effectiveness and communication with key stakeholders. The Associate Director uses safety driven principles to support a just and reliable culture, and is responsible for monitoring project goals and milestones, achieving sustainable positive outcomes from improvement work, and for identifying opportunities to spread standardized best practices. The Associate Director will work closely with leaders across Metropolitan to understand strategic direction and priorities to promote Patient Safety.

Major Responsibilities:
• Help monitor, assure, and coordinate Patient safety activities are in compliance with requirements of regulatory and accrediting agencies identifying safety expectations. Identify and communicate need for corrective actions as appropriate.
• In collaboration with Regulatory, monitor Sentinel Event Alerts and facilitate team to analyze the safety of our organization based on the SE Alert information when needed.
• Responsible for working with Risk Management, legal counsel and others to create and ensure a robust process for monitoring, prevention, event mitigation, event reporting, and triaging.
• Work collaboratively with all levels of the organization including senior leadership to produce measurable improvements in patient safety in both process and outcome.
• Recommend and facilitate change within the organization to improve patient safety based on identified risks.
• Implement and evaluate a regularly scheduled Culture of Safety Surveys-frequency to be determined with leadership.
• Responsible for the RCA and Proactive Risk Assessment team process, including but not limited to initiating a team, creating, and communicating to the team, participation of teams, assuring standard reporting, meeting TJC Sentinel Event requirements and deadlines, ensuring team action items are completed, developing status reporting to RCA Subcommittee, and creating status reports for leadership committees.
• Administratively responsible for planning, management and implementation of patient safety initiatives across the organization, including TJC National Patient Safety Goals, among others, meeting milestones, deadlines, including education and communication plans.
• Lead selected organization improvement teams to stay current with skills.

Minimum Qualifications
1. Master's degree from an accredited college or university in Public Health, Public, Healthcare or Hospital
Administration or a related discipline and two (2) years of clinical experience in coordinating and monitoring
initiatives, activities, and programs related to patient safety and/or quality improvement or performing other
functions directly related to patient safety in a hospital or health care setting; or
2. Bachelor's degree from an accredited college or university in disciplines, as listed in "1" above and three (3)
years of experience, as described in "1" above; or
3. A satisfactory equivalent combination of training, education, and/or experience. However, all candidates must
have a minimum of a Bachelor's degree in disciplines, as listed in "1" above.
For Assignment to Level II:
1. Valid New York State license and current registration to practice as a Registered Professional Nurse or a
Physician Assistant issued by the New York State Education Department (NYSED) or is a Medical Graduate and
two (2) years of experience, as described in "1" of Assignment Level I above; or
2. Two (2) additional years of experience performing tasks, as described in Assignment Level I.
