POSITION SUMMARY

Engage and collaborate with clients presenting for care at the Crisis Center. Provide continuity of care throughout the client's stay at the Crisis Center, developing a discharge plan and providing consistent follow up post discharge, to support and encourage engagement in services. Invite client's participation in the care process during a crisis, communicating clearly re: all options and aligning with the client's preferences whenever possible. Function as an integral member of the Crisis Center interdisciplinary team to maximize care for the clients.

VITAL DUTIES AND RESPONSIBILITIES
  • Welcomes clients to the Crisis Center, conveys they are in a safe place and develops a supportive rapport.
  • De-escalates clients as needed and interacts with clients from a posture of dignity and respect.
  • Provides information and supports clients while making referrals to community services they prefer.
  • Contributes to the Crisis Center's interdisciplinary treatment team decisions to maximize care for clients.
  • Communicates with clients re: all options, aligning with their preferences whenever possible.
  • Collaborates with client in creating and implementing a safety plan.
  • Develops a discharge plan with the client.
  • Provides follow up to clients post discharge, assisting them in accessing services and navigating barriers.
  • Interacts with clients as peers by sharing common experiences.
  • Assists clients in understanding and managing their symptoms.
  • Assists clients in connecting with self-help groups when appropriate.
  • Maintains strict confidentiality at all times.
  • Participates in weekly clinical supervision with appropriately licensed/certified staff.
  • Acts as an advocate for the client regarding needs, preferences, rights and responsibilities.
  • Assist clients in gaining access to local programs and services, as well as tracking of client follow up.
  • Completes electronic and paper medical record documentation as required.
  • Attends staff meetings and crisis debriefings as scheduled.
  • Interacts with clients supportively while performing necessary tasks such as contraband search, close observations, environmental checks, meals, paperwork.
  • Communicates and documents critical incidents to supervisors regarding either employees or clients.
  • Performs other duties as assigned.

EDUCATION AND EXPERIENCE
  • High School Diploma required.
  • Peer Recovery Coach Certification required.
  • CPR Certification is required.
  • CPI certification is required.
  • One year of experience in mental health, peer coaching or related experience is preferred.
  • Must have lived experience and be in long-term recovery (at least 3 years) from mental illness/substance abuse.
