Medical Safety Clinical Specialty position
Required: Knowledge of medication-use safe practices; ISMP initiatives; National Patient Safety Goals; and Process Improvement methodologie
Knowledge of pharmacy information systems and automated dispensing technology.
Highly Desired: Advanced study in pharmacy or health care related field, pharmacy residency, or comparable work experience
Required: 3 years of experience in the area of medication safety specialty practice with familiarity of Institutional Pharmacy services, and medication safety regulatory and legal requirement

- Schedule: Monday-Friday, 8:00AM - 4:30PM, and one 1 weekend rotation every 6-8 weeks

Responsibilities

- Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems.
- Recommend formulary alternatives when appropriate.
- Compound and dispense medications for accurate delivery to patients.
- Provide drug information and education to medical staff and patients.
- Communicate with physicians and other clinicians as necessary to facilitate care.
- Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.
- Assist with academic functions such as serving as a preceptor for pharmacy students.
- May float to provide coverage of pharmacy needs.
- Provide patient counseling when required.
Licensure / Certification / Registration:
- Pharmacist credentialed from the Maryland Board of Pharmacy obtained prior to hire date or job transfer date required.
Education:
- Bachelor's or advanced degree in Pharmacy required.
- Maryland registered pharmacist with either a Doctor of Pharmacy degree with at least 2 yrs of progressive hospital experience with direct patient care responsibilities.
- Completion of Pharmacy Practice Residency training highly desirable
- Working knowledge of Pyxis/Pyxis ES, Sentri 7, Meditech with sterile compounding, antimicrobial stewardship and pharmacokinetic dosing experience.
- Ability to work independently and cooperatively with hospital and pharmacy staff in order to meet departmental and organizational goals.
- Able to handle and manage change in workload and work schedule.
- Membership in ASHP and associated area of practice.
- Board Certification (BCPS or BCACP, BCOP)
- Hospital experience preferred
- PGY1 or PGY2 Residency- with focus towards specific practices eg. Cardiology, ICU, Administration etc.
- Specialty pharmacy experience in a hospital setting
- Knowledge of contemporary hospital/clinical practice and service.
- Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management and specialty evidence based medicine in an area appropriate for clinical assignments.
