Summary:
Provides comprehensive pharmaceutical care to CCH patients, including drug information, therapeutic drug monitoring and pharmacokinetic dosing, renal dose adjustment, therapeutic interchange, IV to PO conversion, and patient education / counseling. Serves as a subject matter expert for the healthcare team regarding drug therapy. Advocates safe medication use. Aligns prescribing, dispensing, administration, and monitoring functions with established policies and procedures for safe practice.

Qualifications
Required Education:
Bachelor's degree is required
Master's degree is preferred
Doctoral degree is preferred
Other: Experience in hospital or similar Pharmacy setting preferred. Strong computer skills essential. Ability to communicate clearly in written and oral forms required.
